Nitric Oxide: The Most Important Medical Discovery Ever?

If there was ever a “miracle molecule” worthy of being called such (which has now been overused thanks to numerous come-and-go fads in the health and fitness industry), it would be nitric oxide (NO). 

When a trio of scientists was awarded The Nobel Prize in Medicine and Physiology in 1998 for their discovery of nitric oxide, which was hailed as “a signaling molecule in the cardiovascular system”.

One of the scientists in this group, Louis J. Ignarro, went as far as to say this:

“There may be no disease process where this miracle molecule does not have a protective role.”

It’s arguably the true anti-aging secret we’ve been looking for all along, as its lack of production and/or availability is directly linked to many of the world’s deadliest diseases of aging.

You simply cannot be fully optimized if your nitric oxide levels aren’t consistently maintained at healthy levels.

All of the above was brought to my attention by Dr. Nathan S Bryan, who has pioneered the field of therapeutic nitric oxide use and published 90 studies on this molecule alone to date.

This article will cover exactly how and why nitric oxide is so essential for human health, the main health benefits it provides, how to naturally boost nitric oxide levels, and the ONLY nitric oxide supplements you should be taking.

What is Nitric Oxide?

Nitric oxide (NO) wasn’t always the rising superstar in medicine — prior to the 1900s, we just knew it as nothing more than a gas that naturally occurs in the atmosphere:

“Nitric oxide is formed from nitrogen and oxygen by the action of electric sparks or high temperatures or, more conveniently, by the action of dilute nitric acid upon copper or mercury.

It was first prepared about 1620 by the Belgian scientist Jan Baptista van Helmont, and it was first studied in 1772 by the English chemist Joseph Priestley, who called it “nitrous air.”

It wasn’t until 1980 that scientists would discover NO was also produced by the human body, let alone serve a fundamental biological purpose:

“Furchgott & Zawadzki (1980) first described endothelium-dependent relaxation, a phenomenon whereby acetylcholine relaxes isolated preparations of blood vessels only if the vascular endothelium lining the vessels is present and intact.

Subsequent studies revealed that acetylcholine and other agents (including bradykinin, histamine and 5-hydroxytryptamine) release a transferable factor (endothelium-derived relaxing factor, EDRF) which is unstable, acts via stimulation of the soluble guanylate cyclase and is inhibited by haemoglobin and methylene blue”

Seven years later, the true identity of ERDF was finally revealed thanks to several identifying tests:

“In 1987, at a symposium in Rochester, Minnesota on Mechanisms of Vasodilatation, Bob Furchgott proposed that EDRF might be nitric oxide (NO).

This proposal was based on the observations that superoxide dismutase (SOD, which removes O2) protected EDRF from rapid inactivation and that haemoglobin selectively inhibited EDRF, as well as on a study of the transient relaxations of endothelium-denuded rings of rabbit aorta to ‘acidified’ inorganic nitrite (NO2) solutions”

For those of you who were able to translate all the science talk above, or have been reading my articles very closely, NO is a vasodilator that improves blood flow:

[Nitric oxide is] a pivotal signaling molecule that regulates blood flow and tissue oxygenation.

NO affects two key aspects of O2 supply and demand: It regulates vascular tone and blood flow by activating soluble guanylate cyclase (sGC) in the vascular smooth muscle, and it controls mitochondrial O2 consumption by inhibiting cytochrome c oxidase.

Abnormalities in NO production and transport in vascular systems lead to numerous cardiovascular diseases, including hypertension, atherosclerosis, and angiogenesis-associated disorders.”

This same process is what allows you to have a stronger immune system, maintain the ability to learn and memorize information, and keep a healthy heart.

To nobody’s surprise — and as I mentioned last year — a lack of blood flow is one of the major contributors to hair loss.

And while nitric oxide is naturally produced by the body in multiple places, the chemical reaction behind its formation is more-or-less the same:

“Endogenous NO is derived from both enzymatic and nonenzymatic sources in and near the vasculature [The vasculature consists of the lumen containing erythrocytes, the erythrocyte-free zone resulting from blood flow, the endothelium, smooth muscle, the nonperfused region, and the capillary perfused region].

The enzymatic formation of NO is catalyzed by nitric oxide synthase (NOS) through a series of redox reactions. In this process, l-arginine is degraded to l-citrulline and NO in the presence of molecular oxygen and nicotinamide adenine dinucleotide phosphate (NADPH).”

Nitric oxide is arguably as important as oxygen if not more important… without nitric oxide, you’re not going to deliver oxygen to the tissues that need it:

“The current convention describes the respiratory cycle as using blood to transport two gases – oxygen and carbon dioxide. Red blood cells pick up freshly inhaled oxygen from the lungs and carry it to cells in the tissues of the body, and bring back carbon dioxide – a waste product of metabolism – to be exhaled from the lungs.

… Prof. Stamler and colleagues argue that the respiratory cycle also involves a third gas – nitric oxide – that controls the release of oxygen from red blood cells into the tissues that need it.

…hemoglobin – the protein in red blood cells that picks up oxygen from the lungs – also needs to carry nitric oxide to enable blood vessels to open and supply the oxygen to tissues.

‘Blood flow to tissues is actually more important in most circumstances than how much oxygen is carried by hemoglobin. So the respiratory cycle is actually a three-gas system.'”

Why is Nitric Oxide Essential for Bodily Health?

I realize the importance of optimal nitric oxide production hasn’t quite landed yet, so let me explain why you needed to start concerning yourself about it YESTERDAY.

First, nitric oxide production naturally decreases with age.

Similar to growth hormone, our bodies just don’t have the capability to keep producing all the good things forever at the highest levels possible.

Some estimates measure our loss of endothelial-produced nitric oxide as roughly 10% per decade starting from our 20s.

But the data is clear: Several studies (here, here, and here) demonstrate a progressive decrease in nitric oxide bioavailability, production, and signaling.

The good news is this age-related decline is not irreversible and can actually be prevented (more on that later)

Second, nitric oxide deficiency is directly connected to accelerating the world’s deadliest diseases.

Blood flow isn’t just the latest interest of the bros and fake gurus on Twitter.

As this deep-dive literature review reveals, the lack of nitric oxide production is integral to the early signaling and continuation of the major causes of death in modern society.

“Studies in experimental models and even humans reveal that constitutive production of nitric oxide (NO) is reduced with aging and this circumstance may be relevant to a number of diseases that plague the aging population.

NO is a multifunctional signaling molecule, intricately involved with maintaining a host of physiological processes including, but not limited to, host defense, neuronal communication and the regulation of vascular tone.”

NO is one of the most important signaling molecules in our body, and loss of NO function is one of the earliest indicators or markers of disease.

Clinical studies provide evidence that insufficient NO production is associated with all major cardiovascular risk factors, such as hyperlipidemia, diabetes, hypertension, smoking and severity of atherosclerosis, and also has a profound predictive value for disease progression including cardiovascular and Alzheimers disease.” 

Obviously, these disease states are extremely complex and other factors aside from nitric oxide production are clearly playing significant parts simultaneously.

But it would be incredibly ignorant to dismiss the one very obvious link between all of these diseases.

I’m not saying we can cure cancer with nitric oxide alone, yet it’s very clear we can do a hell of a lot of good by paying more attention to it.

Fix the poor blood flow and almost everything else will regulate and fix itself.

Top 3 Health Benefits of Nitric Oxide

As of this writing, there are approximately 189,000 published medical papers on the subject of nitric oxide.

It’s abundantly clear there isn’t a single human disease where nitric oxide isn’t involved in some significant capacity.

Therefore, logic dictates there isn’t a single human disease where preventing loss of nitric oxide production and availability through exogenous administration — i.e. giving nitric oxide to the right patient at the right dose — would not be seriously beneficial.

We could reduce the severity of many age-related chronic conditions, if not prevent them entirely.

However… I want to focus on three particular use cases that stand out most to me.

Not so much because of the evidence supporting them, but because they completely change the way we think and understand certain diseases.

Elimination of Erectile Dysfunction (ED)

In case you need a refresher, nitric oxide is arguably the most important part of getting and keeping an erection:

“Nitric oxide (NO) is believed to be the main vasoactive nonadrenergic, noncholinergic neurotransmitter and chemical mediator of penile erection.

Released by nerve and endothelial cells in the corpora cavernosa of the penis, NO activates soluble guanylyl cyclase, which increases 3′,5′‐cyclic guanosine monophosphate (cGMP) levels.

Acting as a second messenger molecule, cGMP regulates the activity of calcium channels as well as intracellular contractile proteins that affect the relaxation of corpus cavernosum smooth muscle. Impaired NO bioactivity is a major pathogenic mechanism of erectile dysfunction.”

The Phoenix, one of my favorite sexual health devices for improving performance, operates on the principle of stimulating nitric oxide release by breaking up plaque buildup within your penis’ blood vessels (more on that here and here).

But when it comes to the PDE5 (phosphodiesterase type 5) inhibitor Cialis, one of my most frequently used agents for stronger erections and a phenomenal pump at the gym, I may have misspoken about its relation to nitric oxide.

One of the studies I quoted to support this point said the following in its conclusion:

“We show that when NO bioavailability is reduced, PDE-5 inhibition can restore vasodilation onset kinetics of exercise-mediated vasodilation via NO-independent cGMP pathways. These data suggest PDE-5 inhibition may improve exercise vasodilation onset kinetics in pathologies where NO bioavailability is impaired.”

Notice how it specifically says the pathway through which a drug like Cialis (or Viagra) can restore blood flow is INDEPENDENT of nitric oxide.

Why is this important?

Without nitric oxide first and foremost being produced in adequate amounts, those drugs are effectively useless.

The quote below from Wikipedia says it best (my emphasis is in bold):

“The endothelium (inner lining) of blood vessels uses nitric oxide to signal the surrounding smooth muscle to relax, resulting in vasodilation and increasing blood flow. 

Sildenafil (Viagra) is a drug that uses the nitric oxide pathway.

Sildenafil does not produce nitric oxide, but enhances the signals that are downstream of the nitric oxide pathway by protecting cyclic guanosine monophosphate (cGMP) from degradation by cGMP-specific phosphodiesterase type 5 (PDE5) in the corpus cavernosum, allowing for the signal to be enhanced, and thus vasodilation.”

Cialis and Viagra do not produce nitric oxide, but rather they work DOWNSTREAM of nitric oxide.

Think of it this way: Nitric oxide is what turns the “switch” on, and Cialis/Viagra keeps the” switch” on (which is why having a painful erection for more than 4 hours after taking either drug is a result of being unable to turn off the “switch”).

You HAVE to dilate the blood vessels within the penis through nitric oxide first in order to get engorgement.

This is exactly why up to 30% of men who are prescribed Cialis/Viagra find it does nothing for their erections.

So what’s the solution here?

You have to restore the body’s nitric oxide production in these patients and give them enough nitric oxide to fully dilate their penile blood vessels. 

By getting to the root of the problem, you can fix and even enhance sexual activity.

“…Sixty-five percent of patients noticed a decrease in visible wrinkles at 4 weeks, 70% by 8 weeks, and 87% by 12 weeks. WOUND HEALING: 

“Twenty-five patients were studied in two centers between December 2018 and June 2020. Prior to utilizing NO serum, a double-blind, placebo controlled (glycerin) safety study was performed on 10 patients. There were no allergic or irritation reactions to NO serum in this population. NO serum was applied to non-healing diabetic ulcers, vascular compromised non-healing wounds, surgical incisions, split thickness grafts, full thickness grafts, regional and free flaps.”

“…the study population showed a more rapid and improved quality of healing in the wounds that were treated with Nitric oxide generating serum as compared to control sites. Rapid reepithelialization, wound contraction, less bruising and edema were commonly seen. Neo-vascularization was more rapid in surgical flap cases. Burn wounds healed dramatically”

The applications of nitric oxide for treating more serious skin diseases are limitless.

How could it not be?

When you improve blood flow, you improve the delivery of oxygen and nutrients to the skin cells so they can restore their function.

Furthermore, cell turnover is much quicker (i.e. old dysfunctional cells are cast aside so new functional cells can be formed).

Optimizing blood flow appears to solve almost every problem in existence, even the one that’s been plaguing us since the start of 2020…

Possible Early Treatment of COVID-19

This final yet critical application of nitric oxide takes us to Dr. Bryan’s second company Nitric Oxide Innovations, a by-the-book drug company subject to a regulatory environment, capital requirements, and FDA approval.

I did mention nitric oxide very briefly at the start of the COVID-19 pandemic when I was working 24/7 to find early treatment interventions but never paid any attention to it.

Turns out this was a huge miss on my end!

Not only was Dr. Bryan quicker than I to consider nitric oxide as a major culprit behind severe COVID-19 cases, he also made a much stronger case for why it plays a pivotal part in how SARS-CoV-2 propagates COVID-19 in the human body:

“Pathology is taught during the first and sometimes second year of medical school. 

The primary textbook used in many medical schools is Robbins Pathology.

On page 58, it clearly states:

‘Well-vascularized tissues are more resistant to infections and capable of localizing/containing offending agents.

By contrast, poorly vascularized tissues are relatively inefficient in responding to inflammatory stimuli.’

This means that if you have good circulation and blood flow to every tissue in the body, then this allows your immune system to mobilize a strong defense against any invading pathogen.

If you don’t have good blood flow and circulation, the infection takes hold and makes you sick and can sometimes kill you.

This is basic physiology.”

Expanding on this, nitric oxide is also produced by our immune cells to help the body defend itself through the elimination of invading pathogens.

This is what a healthy immune system would do to prevent the virus from rapidly replicating and propagating throughout the body.

If your body can’t make enough nitric oxide and fail to make it fast enough, you get vascular inflammation and a sharp drop in oxygen saturation.

Once you’re in the hospital, it’s game over… even when equipped with a stream of 100% oxygen gas, you can’t improve oxygen saturation because you still need nitric oxide.

It only took an additional month of collecting data from patients to confirm Dr. Bryan’s hypothesis: The people who were more susceptible to severe COVID-19 illness and death were the same people suffering from various co-morbidities like old age, diabetes, dementia, and various cardiovascular problems.

In other words… all the same people who suffer from disease states where their bodies have lost the ability to generate nitric oxide!

Dr. Bryan was quick to start developing a novel nitric oxide treatment for COVID-19 but noticed that all the early treatment studies published before him made one fatal mistake:

“The solution to COVID-19 worldwide pandemic may well be nitric oxide.

In fact, there are a number of clinical studies using inhaled nitric oxide in critically ill patients.

There are a few problems with this approach many of the hospitals are using. 

Number 1, it may be too late to introduce nitric oxide in ventilated patients.

We need nitric oxide prior to the escalation of respiratory distress.

Number 2, administration of too much nitric oxide or for too long can cause methemoglobinemia which will further compromise the oxygen-carrying capacity of the blood.

It is better to be proactive instead of reactive. Furthermore, we need systemic increases in nitric oxide, not just in the airways and pulmonary circulation.”

He even called out the unwise use of antacids (a.k.a. proton pump inhibitors) as they were later found to be associated with an increased risk of COVID-19 infection… which isn’t a shock to anyone who understands these same drugs inhibit nitric oxide production by neutralizing stomach acidity.

And to really show you how using nitric oxide for respiratory illnesses is an absolute no-brainer, check out this quote from a scientific review published in March 2022:

“The correlation between NO levels and respiratory viral disease severity was first noted by Pedja et al., which reported that levels of NO oxidation products in serum were higher in patients that survived acute respiratory distress syndrome caused by H1N1, in comparison with non survivors. H1N1 infection is also known to increase exhaled NO levels.

Later on, following the emergence of SARS-CoV’s predecessor, SARS-CoV-1, Akerstorm et al. found in in vitro experiments that incubating cells with the direct NO donor S-nitroso-N-acetylpenicillamine (SNAP) could inhibit the replication of the virus. Successful containment of the SARS-CoV-1, that resulted in a much more lethal disease than COVID-19, led to the abandonment of further research of this therapeutic pathway for the treatment of SARS.”

Forget about the early treatment drugs starting with the letters “H” and “I” (if you know you know)… we’re talking about a therapeutic treatment option that is vastly superior for both COVID-19 and any respiratory infection.

In fact, Dr. Bryan filed an Investigational New Drug (IND) application for his unique nitric oxide formulation in May 2020 and got it approved a month later… not a peep in the world from anybody in the medical community.

It’s literally the world’s best-kept secret, and here’s what Dr. Bryan was willing to share with me about it…

  • NOviricid is an orally-disintegrating nitric oxide drug (a lozenge) that restores and repletes production of nitric oxide

This section can easily be written by summarizing Dr. Bryan’s book “Functional Nitric Oxide Nutrition”.

Fundamentally, there are only two things you need to do.

The first thing is to STOP doing the activities that sabotage your body’s ability to adequately produce nitric oxide:

The second thing is to START doing the activities that are clinically proven to enhance nitric oxide production:

*Dr. Bryan wrote a book focusing specifically on red beets and their health benefits that extend far beyond promoting nitric oxide production. 

As you can probably tell, these are the exact same things you would do for stimulating neurogenesis and improving mitochondrial health!

And from Dr. Bryan’s experience, the poison is dictated by the dose.

In the context of nitric oxide, saying too much nitric oxide is dangerous (which would lead to an unsafe drop in blood pressure) is akin to saying drinking water is dangerous.

Yes, it is technically possible, but the only way you would die from drinking too much water is if you forced an inhuman amount into your body within an extraordinarily short period of time.

Before you go buying the best-selling supplement on Amazon that promises to “200x your nitric oxide production”, you need to know that not all nitric oxide supplements are equal to one another.

Dr. Bryan has compiled a list of the types of supplements that won’t work as promised, and why.

He talks about them in two blog posts (here and here), but here is the 30,000-foot overview:

  • Products containing L-citrulline or L-arginine — most people already have adequate amounts of both amino acids, but the major problem in people deficient in nitric oxide is the loss of the ability to convert L-arginine into nitric oxide due to a functional error with the enzyme NOS (nitric oxide synthase)
  • Products containing nitrates — We cannot metabolize this molecule as it is inert, and need the presence of nitrate-reducing bacteria in order to convert nitrate into nitrite and then to nitric oxide (i.e. it only solves part of the problem and won’t work for enough people)
  • Products based around beet root — Almost every beet product on the market does not contain any amount of detectable nitrate when tested in a lab, let alone have a standardized amount of nitrate listed on the label
  • Liquid products labeled as nitric oxide — nitric oxide does not survive in liquid form
  • Gummies and chews — no detectable nitrite or nitrate, and no detectable nitric oxide upon consumption

In contrast, here’s exactly what you SHOULD look for in a reliable nitric oxide supplement:

  • Restores the bacteria in your mouth that are responsible for producing nitric oxide
  • Restore the ability of nitric oxide synthase to convert L-arginine into nitric oxide
