It happens gradually and then suddenly:

  • You wake up a little more tired than you were a year ago
  • Your waist gets a little thicker despite eating “pretty well”
  • Your energy crashes after lunch
  • Your testosterone starts to drift downward
  • Your blood pressure creeps up
  • You need more caffeine to feel normal

One by one, each change seems small enough to ignore.

Until one day, your doctor tells you the bad news:

You have prediabetes (or Type 2 diabetes), fatty liver disease, an increased risk of cardiovascular disease, and a few other things for good measure.

Sometimes all at the same time! 

The diagnosis feels sudden, but only because your metabolism has been deteriorating for years.

All the warning signs were in plain sight this entire time, and the reason why you couldn’t see or hear them is because most physicians simply aren’t looking for them.

Today, I AM going to show you where metabolic dysfunction actually begins and why standard blood work often misses it.

From there, you’ll learn how to recognize the problem long before it becomes a disease (or several).

The Sick-Care System Is Built to Fail You

Conventional medicine does not focus on helping you get fully optimized.

Your doctor is trained to catch full-blown Type 2 diabetes when it appears, instead of the decade-long metabolic deterioration that precedes it.

For example: The diagnostic threshold for Type 2 diabetes is a fasting glucose reading above 126 mg/dL, and the “prediabetes” range starts at around 100 mg/dL.

But research consistently shows insulin resistance, the root-cause driver of metabolic dysfunction, can be fully entrenched years before fasting glucose ever climbs out of the normal range.

This is a systemic failure leaving millions of men chronically unwell and undertreated.

All while remaining misled about the actual and current state of their health.

What Metabolic Dysfunction Actually Is

Metabolic dysfunction is about so much more than blood sugar.

It is a breakdown in the body’s ability to process, store, and utilize energy efficiently.

This is fundamentally driven by insulin resistance, where cells stop responding properly to insulin’s signaling.

To back-track a bit: Insulin is the primary anabolic hormone responsible for shuttling glucose into cells.

When cells become insulin-resistant, the insulin receptor substrate (IRS-1) pathway is downregulated.

The pancreas compensates by pumping out MORE insulin to force the same effect, and this leads to a condition known as hyperinsulinemia.

Where the story takes a darker turn is that chronically elevated insulin does the following:

  • Drives fat storage
  • Suppresses fat burning
  • Promotes systemic inflammation
  • Disrupts hormonal signaling
  • Accelerates aging
  • Increases the risk of cardiovascular and neurodegenerative disease

Insulin resistance is the upstream driver of nearly every major chronic illness afflicting modern civilization.

And it can wreak havoc on your metabolism for years before a fasting glucose test ever catches it.

The Hidden Signs Your Doctor Dismisses

The conditions I just mentioned are biological signals of metabolic dysfunction that most physicians are not trained to connect.

Here are some other subtler signs you may not be aware of…

Stubborn abdominal fat is a direct downstream effect of hyperinsulinemia.

Insulin is profoundly lipogenic because it signals fat cells to store energy and actively blocks lipolysis

If you cannot lose your midsection no matter how hard you train, look at your insulin first and understand that visceral fat accelerates aging at the cellular level.

Chronic fatigue and post-meal energy crashes mean that glucose is not being efficiently utilized by cells.

When mitochondrial function is impaired by metabolic dysfunction, cellular energy production suffers at a fundamental level.

Brain fog and poor concentration are among the most underrecognized signs.

Insulin resistance in the brain, now being studied under the framework of “Type 3 diabetes” in relation to Alzheimer’s, impairs neuronal glucose uptake and accelerates cognitive decline.

Poor sleep quality and sleep apnea are bidirectionally linked to insulin resistance.

Disrupted sleep raises cortisol, which raises blood glucose and drives further insulin resistance, and it becomes a self-reinforcing spiral.

Skin tags and acanthosis nigricans (darkened, velvety skin patches at the neck and armpits) are direct dermatological markers of chronically elevated insulin.

If your doctor is not asking about either of these two things, they are not looking at the full picture.

High triglycerides with low HDL on a standard lipid panel is one of the clearest metabolic fingerprints in conventional labs.

This pattern reflects impaired lipid metabolism downstream of insulin resistance, and it often precedes elevated fasting glucose by years.

Elevated blood pressure without an obvious cause is frequently linked to hyperinsulinemia via sodium retention mechanisms in the kidney and impaired nitric oxide signaling in blood vessels.

Another “hidden” signal your doctor probably isn’t looking for.

Finally, hormonal imbalances including low testosterone in men are directly connected to metabolic dysfunction.

Excess insulin drives aromatase activity, leading to the conversion of testosterone to estradiol.

Metabolic health and hormonal health are inseparable, and I can confidently say this from direct experience and years of working with men in this space.

The Lab Markers That Actually Reveal the Truth

Here is what an optimization-minded physician will actually order, and I break down the complete panel in my newest book, Living Leaner Longer Stronger.

  • Fasting insulin should ideally be below 5 uIU/mL, yet most conventional labs flag anything under 25 as “normal”
  • HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is calculated from fasting glucose and fasting insulin, where a score above 1.5 suggests early insulin resistance and above 2.0 is significant
  • The triglyceride to HDL ratio is ideal below 1.0 in mg/dL units, and above 2.0 is a strong signal of metabolic dysfunction
  • HbA1c (glycated hemoglobin) reflects average blood glucose over 90 days, with an optimal range of 4.8 to 5.2%, so do not accept “you’re not diabetic yet” as an answer if you sit at 5.6%
  • Fasting glucose should never be read alone because you have to look at it alongside insulin to get the real picture, which is exactly why I’m such an advocate of continuous glucose monitoring for non-diabetic adults
  • ApoB and LDL particle number matter because small dense LDL particles are a downstream metabolic marker that is far more predictive than standard LDL cholesterol
  • Uric acid is an underappreciated marker of metabolic dysfunction and fructose overconsumption

The Hormonal Connection You Cannot Ignore

Metabolic dysfunction and hormonal dysfunction are part of the same conversation.

Chronically elevated insulin directly suppresses sex hormone-binding globulin (SHBG), and while low SHBG on TRT has its own nuances, chronically crushed SHBG driven by insulin is a major red flag.

Hyperinsulinemia drives elevated aromatase enzyme activity in adipose tissue, which you now know accelerates the conversion of testosterone to estradiol.

The end result is a man with low testosterone, elevated estrogen, increased fat mass, poor recovery, low libido, and brain fog… all being driven by insulin resistance.

This is why I always tell any man pursuing that testosterone optimization that failing to addressing metabolic health first will ultimately lead to a broken foundation.

My wife Monica has written extensively about this pattern in women, where insulin resistance intersects with estrogen dominance and progesterone imbalances to create a hormonal dysfunction cascade that most conventional OB-GYNs never connect to metabolic health.

The solution starts in the same place regardless of sex: Fix the metabolic house first!

Safety, Risks, and What to Watch For

NEVER assume you are metabolically healthy because you feel “all right.”

Metabolic dysfunction is largely asymptomatic in its early stages.

By the time symptoms become severe enough to prompt a conventional diagnosis, you may have already lost a decade of optimal function.

A few critical safety considerations to keep in mind:

  • Do not self-diagnose or self-treat insulin resistance without proper lab work (and ideally guidance from an optimization-minded physician)
  • Aggressive carbohydrate restriction without adequate protein and fat calibration can create its own share of hormonal and metabolic problems, particularly in already-stressed individuals.
  • If you are on any medications, particularly blood pressure lowering or glucose-modulating drugs, any dietary or lifestyle intervention should be discussed with your clinician before going forward with any protocol.
  • Fasting protocols, while powerful tools for improving insulin sensitivity, are contraindicated in individuals with certain hormonal disorders, a history of eating disorders, or adrenal dysfunction.

In summation: Work with someone who sees optimization as the true end goal.

The men who age the best, perform the best, and feel the best are the ones who refused to accept “normal” as a standard and went searching for the real answers.

Your doctor’s #1 priority in the sick-care system is simply to keep you out of a hospital bed.

But if you want to live a fully optimized life, you MUST operate at a completely different standard.

Metabolic health is best defined as a fully functioning and energy-efficient system where insulin sensitivity is high.

One that supports every other aspect of performance and longevity.

Put another way, it is much greater than the mere absence of disease.

Here’s how to get started on the right path:

  • Get the right blood work done — understand your HOMA-IR, your fasting insulin, and your triglyceride-to-HDL ratio
  • Find a physician who speaks the language of health optimization
  • Look at your body composition honestly, and dial in the insulin-controlled lifestyle that keeps every one of these markers where it should be.
  • Pay attention to your energy levels – are you sleeping well at night, is your cognitive clarity sharp, etc.

All of these things are biological signals telling you the truth about where you actually stand.

You have the right to those truths, but it is your responsibility to act upon them.
