The moment you tear a muscle or strain a tendon, your body sends out a dispatch call.

Thymosin beta-4 is one of the signals that goes out first, recruiting repair cells to the damaged tissue and supporting new blood vessel formation.

Alongside other processes, the true end goal is preventing inflammation from running out of control.

There’s just one problem: Your body can only produce a finite amount of thymosin beta-4.

It will eventually reach a point — due to decades of chronic inflammation and/or accumulation of wear and tear — where the dispatch system falls behind.

And if this doesn’t happen gradually, it can also happen suddenly following a serious injury.

That’s when you have to call in the reinforcements, which is where TB-500 comes into the conversation.

In my newest book Living Leaner Longer Stronger, I described a workplace analogy where BPC-157 is the specialist who repairs the damaged tissue while TB-500 is the project manager making sure the entire repair crew arrives on time.

This critical yet little-known distinction explains why most TB-500 dosing advice online is wrong.

What I AM going to do in this article is reveal the protocols I’ve actually published (and used) and where generic dosing charts fall apart.

That way, you can see what truly matters if you’re serious about tissue repair.

Quick Takeaways

  • TB-500 works through cell signaling and not through receptor saturation, which is all the more reason why using more DOES NOT automatically mean you’ll get better results.
  • My published loading protocol for active injury recovery is 2.0-2.5 mg twice weekly for 4 to 6 weeks, followed by a lower maintenance dosing protocol.
  • The daily protocol found in my Peptide Cheat Sheet calls for 500 mcg every morning at 8 weeks on and 8 weeks off, while Monica runs women at 500 mcg four times weekly on an “8 weeks on and 4 weeks off” cycle.
  • TB-500 has a relatively long half-life, and that’s the primary reason why it can be dosed less frequently than most peptides while still remaining effective.
  • Inject TB-500 subcutaneously, but not directly into the site of injury as TB-500 circulates throughout the body.
  • Most people fail for numerous reasons when they use TB-500: If they’re not already running the wrong protocol for their intended goal,  they either start too late or skip injections.

What TB-500 Actually Is

Thymosin beta-4 (Tβ4) is a naturally occurring peptide found in nearly every cell in your body, first isolated and sequenced decades ago by Allan Goldstein’s research group.

TB-500 is the synthetic version of the active portion of Thymosin beta-4, and it just so happens this fragment is more practical to use than the entire protein.

It’s something I pointed out years ago in my book The Testosterone Optimization Therapy Bible.

Its primary mechanism involves actin, the structural protein cells use for migration within the body.

Here’s why this mtters…

Thymosin beta-4 is an actin-sequestering protein that moonlights as a tissue repair signal, regulating the cytoskeletal reorganization repair cells require to migrate into damaged tissue.

In a rat wound model, Tβ4 accelerated wound healing to the point where it sped up re-epithelialization and drove the stimulation of both collagen deposition and new blood vessel formation.

Tβ4 also dampens TNF-driven NF-kB activation, one of the master switches behind inflammatory signaling in soft tissue.

The point I’m attempting to make is this: TB-500 is NOT a growth factor, and it does not work like a hormone binding a receptor with a clean dose-response curve.

Most biohackers fail to understand this fact and it explains exactly why the generic dosage charts for TB-500 found online are almost always oversimplified.

Why Most People Get TB-500 Dosing Wrong

Most TB-500 protocols floating around the internet were designed for the express purpose of convenience.

Give people one easy number they can remember and them figure out the rest.

This short-sighted thinking has led to numerous mistakes, and the four most common ones I’ve seen over the years are as follows:

  1. Running the wrong protocol for the goal — using a light daily maintenance dose for an acute injury that needed a loading phase, or vice versa
  2. Stopping the loading phase too early — before the cumulative signaling effect has meaningfully influenced tissue repair.
  3. Injecting directly into the injured area — which is unnecessary because TB-500 works systemically once absorbed).
  4. Treating TB-500 as standalone therapy —  failing to stack it with proper mechanical loading, sleep, and other regenerative peptides like BPC-157.

The sick-care system doesn’t want you to understand nuance, but it turns out that neither do our fellow biohacking bretheren.

They just simply want you to follow a chart and not ask too many prodding questions.

My Published TB-500 Dosage Protocols

These are the protocols from my two books Living Leaner Longer Stronger and The Testosterone Optimization Therapy Bible, along with updated information from my Peptide Cheat Sheet.

They come from decades of my own self-experimentation and widespread use across the underground bodybuilding communities, combined with a deep body of existing animal research.

Protocol Dose Frequency Duration
Active injury loading 2.0 to 2.5 mg 2x per week 4 to 6 weeks
Chronic tissue support Lower maintenance dose After the loading phase Ongoing or as needed
Daily protocol (men) 500 mcg Every morning 8 weeks on, 8 weeks off
Daily protocol (women) 500 mcg 4x per week, AM 8 weeks on, 4 weeks off
BPC-157 / TB-500 blend 250 mcg each Morning and night, daily 8 weeks on, 8 weeks off

New users should start conservatively (ideally under practitioner supervision) and let the results dictate the adjustments to be made, if any.

While you may see 5-10mg per week recommended on peptide forums for acute injuries, that dose is far above anything I have ever published or recommended in my lifetime.

Ang again: With a signaling molecule, MORE IS NOT BETTER!

Why Generic Protocols Fall Short

Generic protocols assume every user has the same injury within the same type of tissue and is working with the same baseline level of inflammation.

And there are several reasons why this black-and-white thinking is flawed.

For starters, TB-500 works through systemic signaling.

In other words, its effects travel through the bloodstream instead of staying confined to the injection site like a corticosteroid shot would.

So a powerlifter with a chronic tendon issue requires a fundamentally different strategy from a woman managing a minor soft tissue strain.

The powerlifter needs the full loading phase, whereas the woman may be better served by Monica’s four-times-weekly daily protocol.

Generic protocols also ignore the timing of the dose.

In my experience, TB-500 does its best work when you start using it early in the repair process… NOT several weeks after the injury has already scarred over.

When you start using it too late, you’re working against a biological window that has already started closing.

How to Dose TB-500 Based on Your Goal

The biggest lesson I’ve learned after years of self-experimentation is that consistency across the loading phase matters more than the size of any single dose.

Skipping injections and expecting the same outcome is like skipping half your training sessions and expecting the same strength gains.

Assuming you don’t have a problem with following a schedule, here is what you should do with TB-500 based on your exact recovery demands…

  • For an acute soft tissue injury: Run the full loading protocol of 2.0 to 2.5 mg twice weekly for 4 to 6 weeks, then step down into a lower maintenance rhythm.
  • For general systemic recovery and tissue resilience: The daily 500 mcg cheat sheet protocol makes far more sense than chasing “more is better.”
  • For tendons and ligaments specifically: Pair it with BPC-157, which is the stronger tool for connective tissue, and follow my BPC-157 dosing guide for the other half of the stack.

Inject subcutaneously with an insulin syringe, and rotate sites well away from the injury (regardless of which option from the list above you choose).

Some physicians occasionally use intramuscular injections for specific muscular injuries, but subcutaneous administration remains the standard.

And because TB-500 has a relatively long half-life, it can be dosed less frequently than most peptides while maintaining its potency.

Myth vs Reality

Myth: More TB-500 means faster healing.

Reality: Cell signaling pathways have saturation points, so excessive dosing doesn’t accelerate repair and only leads to product waste.

Myth: You have to inject TB-500 where it hurts.

Reality: TB-500 circulates throughout the body after a subcutaneous injection and supports repair wherever the signal is necessary.

Myth: TB-500 works identically for tendons, muscle, and skin.

Reality: Vascularity differs dramatically between tissue types, which changes how quickly systemic signaling reaches the target area.

Myth: You’ll feel TB-500 working immediately.

Reality: Meaningful tissue remodeling is a weeks-long process, and does not happen acutely over a 24-48 hour period.

Myth: TB-500 replaces rehab and mechanical loading.

Reality: Peptides support the biological environment for repair, but they do not replace the necessary progressive loading for tendons and connective tissue to make a complete recovery.

Safety, Risks, and Contraindications

I’m not your physician and nothing in this article should constitute medical advice, but I can be direct with you as someone who has spent more than two decades in the space of therapeutic peptides.

The lethargy and head rush associated with some peptides are not widely reported with TB-500.

The far bigger risk you should be concerned about is what’s actually in the vial you’re using.

TB-500 is sold for research purposes only, which means sourcing quality and purity control are 100% your fault and responsibility:

  • Unvetted peptide vendors frequently sell underdosed or contaminated product, so learn how to evaluate peptide quality while demanding traceable batch and lot numbers with a third-party certificate of analysis.
  • Reconstitution errors and poor sterile technique increase your risk of infection, so follow my guide on storing reconstituted peptides and use bacteriostatic water every time.
  • Anyone with active cancer and/or a history of malignancy should be especially cautious as the same cell migration and angiogenesis processes that drive tissue repair are also involved in tumor growth.

My #1 sourcing recommendation will always be BioLongevity Labs, where every research peptide is verified for purity through double and triple third-party certification.

And last but not least, work with an optimization-minded physician who understands regenerative medicine.

A Special Note for Women

The conversations about TB-500 are almost always framed around men, but soft tissue repair matters just as much for women who are managing training injuries or dealing with postpartum recovery.

Monica’s protocol on The Woman’s Peptide Cheat Sheet runs TB-500 at 500 mcg four times weekly in the morning, adhering to an “8 weeks on and 4 weeks off” cycle.

Her BPC-157 and TB-500 blend runs 500 mcg once or twice daily on the same “8 weeks on and 4 weeks off” schedule.

Long story short: If you’re a woman considering TB-500, don’t copy a male bodybuilder’s loading protocol and assume it translates.

Take Ownership of Your Protocol

Generic TB-500 dosage charts are a starting point, albeit not a very good one.

An intelligent dosing protocol will factor in your injury type, the tissue(s) involved, when you’re starting recovery, and your end goals.

Nobody in the sick-care system is going to teach you this since peptide optimization isn’t part of their reimbursement model.

And truthfully told, most biohackers could care less about your recovery.

The responsibility falls entirely on you to get educated and source your peptides properly.

Where possible, work with a practitioner who prioritizes regenerative optimization over symptom management.

For the full compound breakdown, read my guide covering all the TB-500 benefits guide.

As for anyone who is in the middle of recovering from an operation, my post-surgery recovery protocol will show you how TB-500 fits alongside BPC-157.
