Most people in your shoes will do absolutely nothing productive in that window of time.

They “brace for impact” while feeding their FEARS with random searches on Google.

Then the procedure happens, the medical system hands them opioids and a recommendation to get as much rest as possible.

If you’re lucky, a follow-up appointment six weeks out from your surgery will be scheduled.

At best, this is a form of managed deterioration.

But the smart people are already planning ahead of their surgery to ensure they return to full capacity as quickly as possible.

And you can bet your bottom dollar they’ll be tapping into the best therapeutic peptides for post-surgery recovery.

What I AM laying out here is the framework I use and have personally tested on myself, both before AND after the surgery, to get back to peak performance faster than what conventional medicine can offer you. 

Bonus points for the reduced inflammation and tissue protection that comes with my protocol.

Why Surgery Recovery Is Broken by Design

The sick-care system, in case you haven’t already, is built exclusively around standardized care that pertains to complication prevention and liability management.

Optimized healing is NOT a priority for them.

The surgeon’s recommendation to rest up for six weeks is based on healing timelines and population-level risk.

Which is a far cry from the maximum regenerative potential of YOUR biology.

You can influence active biological processes such as angiogenesis, fibroblast proliferation, and nitric oxide signaling with the right inputs and a solid understanding of mechanism-based physiology.

What Prehab Is and Why It Changes Everything

Prehab is the deliberate preparation of your healing infrastructure BEFORE the day of the surgery so you can enter the procedure in the strongest physiological state possible.

If we’re being real with one another, most people start off from a losing position:

  • They are highly inflamed
  • They are nutritionally depleted
  • They are under-muscled
  • Their internal physiology suffers from suppressed growth factor activity

The condition of your biology BEFORE surgery matters more than anything you do in the first week after.

Don’t believe me?

One meta-analysis of preoperative prehabilitation found preparing patients before surgery measurably reduced postoperative morbidity.

We’re talking about REAL conventional human trial data inside the confines conventional surgical medicine.

So it’s almost shocking that despite mainstream medicine having full knowledge of the benefits of prehab work, they don’t apply it aggressively enough.

Meanwhile, I’ve watched men and women in our community cut their projected recovery timelines drastically by taking the pre-surgical window seriously.

BPC-157 After Surgery: The Anchor of Your Protocol

BPC-157 (Body Protection Compound 157) is the single most important peptide for surgical recovery.

It is a synthetic pentadecapeptide derived from a protective protein found in human gastric juice.

Preclinical research has explored its use across tendon, ligament, muscle, bone, nerve, and gastrointestinal tissue.

And if you want to dive into all those individual applications even further, there’s a systematic review of existing work in orthopaedic sports medicine published in 2025 that catalogs this body of work.

What makes BPC-157 so interesting for surgery is the number of repair mechanisms it touches at once:

  • Drives angiogenesis through VEGFR2 activation, pushing new blood vessels into tissue that needs perfusion to heal.
  • Promotes fibroblast outgrowth and migration, which is exactly what rebuilding connective tissue requires.
  • Modulates nitric oxide signaling, which matters for blood flow and ischemic stress around a surgical site.
  • Possesses none of the immunosuppressive tradeoffs you would have to take on with a conventional anti-inflammatory drug.

When it comes to BPC-157, you DO NOT have to inject it into or near the surgical site.

It works systemically once absorbed, circulating and supporting repair wherever the signal is needed.

Something I explained in great detail within the peptides chapter in my newest book Living Leaner Longer Stronger.

Putting a needle near a fresh incision leads to taking on a potential infection risk in exchange for a potential added benefit that doesn’t really exist.

For that reason, you should be rotating your injection sites far away from where the surgery is being performed.

BPC-157 Dosing

I generally recommend 200 to 300 mcg twice daily for soft-tissue recovery, and up to 500 mcg twice daily for acute injuries.

My Peptide Cheat Sheet runs 500 mcg morning and night, every day, on an 8 weeks on and 8 weeks off cycle.

My wife Monica’s protocol for women is 250 to 500 mcg once or twice daily, 8 weeks on and 4 weeks off.

TB-500: The Underrated Partner

TB-500 is criminally underused in the post-surgical context.

It is a synthetic version of the thymosin beta-4 fragment, an actin-sequestering protein that moonlights as a tissue repair signal in nearly every cell in your body.

And its mechanisms are distinct from BPC-157, which is exactly why they stack.

  • It supports the actin dynamics underlying cell migration into damaged tissue.
  • It dampens TNF-driven NF-kB activation and downstream inflammatory signaling, which spikes up rapidly after surgical trauma
  • It has documented roles in cardiac and vascular repair, and this is particularly important once surgical stress hits your systemic circulation

But just like BPC-157, it gets distributes systemically upon administration and this also makes site-specific injections unnecessary here too.

TB-500 Dosing

For active recovery, the loading approach I published in Living Leaner Longer Stronger is 2.0 to 2.5 mg twice weekly for 4 to 6 weeks, followed by a lower maintenance dosing protocol.

Alternatively, my Peptide Cheat Sheet calls for 500 mcg every morning on an 8 weeks on and 8 weeks off cycle, while Monica runs women at 500 mcg four times a week.

As for the peptide forums where 5mg to 10mg per week is being floated around as a recommendation…

That is above anything I have ever published, and more is not better with a signaling molecule.

Therefore, start inside the ranges I listed above and let results dictate the adjustments you make with your physician.

The Prehab Protocol: Before You Go Under the Knife

What you do in the 2 to 4 weeks before surgery matters enormously.

To set you on the right path, here is the framework to discuss with your optimization physician and clear with your surgical team when it comes to the best timing…

Peptide Preparation

BPC-157 injected subcutaneously at 250 to 500 mcg once daily, started 2 to 4 weeks out.

TB-500 injected subcutaneously at 2.0 to 2.5 mg twice per week across that same window.

Stop using these peptides when your surgical team tells you to stop.

And for the love of all that is holy, DO NOT start a brand new injectable in the final days leading up to a surgical procedure.

Hormonal Optimization

Walk in with your testosterone optimized, because you are about to endure a catabolic event and the muscle you carry into the procedure will act as the buffer you spend getting out of it.

Growth hormone secretagogues like CJC-1295 and Ipamorelin can build a stronger anabolic environment in the weeks beforehand.

Fix your hormones first and add in the peptides second (i.e. the same hierarchy I apply to everything else).

Nutritional Priorities

Collagen or gelatin at 15 to 20 grams with vitamin C, since vitamin C-enriched gelatin taken before loading increases collagen synthesis markers in human subjects.

Verify and correct your zinc and vitamin D levels before the procedure.

Push your protein to at least 1 gram per pound of bodyweight if you haven’t already done so.

Lifestyle Non-Negotiables

Maximize sleep quality, given your largest growth hormone pulse happens during phases of deep sleep.

Cut alcohol entirely; it is a cellular toxin and there is no version of any prehab or rehab protocol where it serves you.

Minimize processed seed oils and high-glycemic carbohydrates to lower the inflammatory load you carry into your surgical procedure.

The Rehab Protocol: The First 12 Weeks Post-Surgery

Before you continue, please remember that everything detailed below is subordinate to your surgical team’s clearance.

Days 1 to 14: Acute Phase

BPC-157 at 500 mcg twice daily, injected well away from the surgical field.

TB-500 at 2.0 to 2.5 mg twice weekly, injected in the same way as BPC-157.

Prioritize your sleep hygiene aggressively, as every hour of deep sleep is a repair signal you cannot buy anywhere else.

Finally, minimize NSAIDs use where possible.

A systematic review of perioperative NSAID administration after orthopaedic procedures — and a separate review of NSAID effects on bone and tendon healing — both raise real concerns about blunting the inflammatory cascade that initiates the repair cascades.

Use them when the level of pain you experience calls for it, and talk to your surgeon instead of quietly stopping something they prescribed to you.

Weeks 3 to 8: Remodeling Phase

BPC-157 at 250 to 500 mcg once daily.

TB-500 at 2.0 to 2.5 mg twice weekly through week 6, followed by tapering to a lower maintenance dose.

Start progressively loading the repaired tissue exactly as your clinical team advises.

Reintroduce a resistance training stimulus as early as possible (and under supervision), due to the fact mechanical loading is what “tells” new collagen how to organize.

Weeks 9 to 12: Consolidation Phase

BPC-157 at 250 mcg daily as your maintenance dose, while respecting the 8 week cycle structure.

Continue to rebuild your full training stimulus progressively, rather than ramping back up to 100% intensity from the beginning.

Reassess your blood work (Cortisol, testosterone, IGF-1, inflammatory markers, etc.).

A Special Note for Women

In the Woman’s Peptide Cheat Sheet designed by my wife Monica, it runs BPC-157 at 250 to 500 mcg once or twice daily and TB-500 at 500 mcg four times weekly (both on an “8 weeks on and 4 weeks off” structure)

Her guiding principle is the one that belongs on any peptide protocol: Start low, go slow, and choose one goal at a time.

Estrogen also plays a meaningful role in collagen synthesis, so any woman entering surgery with suboptimal hormonal status should have that conversation with a knowledgeable hormone physician before the day of the surgery.

Safety, Risks, and What You Must Know Before Starting

Nothing in this article is medical or legal advice, and should not be considered as a replacement for a real relationship with a peptide-literate practitioner and a surgeon who knows exactly what you are taking.

BPC-157 and TB-500 do not have completed human clinical trials in the traditional pharmaceutical sense, but the absence of Phase III trials reflects patent economics and should not be taken as a sign of danger.

While the mechanistic and animal data are compelling, the real-world database from decades of athlete and biohacker use is substantial.

Nevertheless, the non-negotiables around surgery are as follows:

  • Tell your surgeon and anesthesiologist about every drug, peptide, hormone, supplement, and compound you are taking with no exceptions
  • Follow their instructions on what gets discontinued and when (bleeding risks and potential anesthesia interactions take priority over your self-created protocol)
  • Never inject anything into or near the surgical field, and DO NOT inject anything without proper sterile technique
  • If you have an active or previous malignancy, the use of angiogenesis-driving compounds raises legitimate concerns that must be brought up with your oncology team before the surgery
  • For the competitive athletes reading me, BPC-157 is treated as a doping agent and is actively targeted in doping control testing (in other words, a surgical recovery protocol can end a competitive season if you are getting regularly tested)

Sourcing is its own safety issue because a mislabeled or contaminated injectable going into a body that just underwent surgery will most likely lead to an unfavorable outcome.
