Fasting Insulin: The Most Important Biomarker Nobody Measures

Every year, millions of people get a standard metabolic panel showing “normal” glucose and A1C numbers, and walk out of the doctor’s office believing they are metabolically healthy.

Meanwhile, their bodies have been silently on a crash course toward insulin resistance.

Fasting insulin is the single most predictive metabolic biomarker available to us today, and the conventional medical system almost never orders it.

Sometimes by accident, but sometimes on purpose as well.

Regardless of the intention, it’s a systemic failure with real consequences for your long-term health.

These consequences encompass your hormone levels, body composition, cognitive function, and longevity.

I am going to tell you exactly what fasting insulin reveals, along with why the “normal” reference range is a dangerous joke (and where you actually have to be).

Why Your Doctor Isn’t Testing For Fasting Insulin

The standard of care in conventional medicine is designed to identify disease when it appears.

Not to prevent the dysfunction before the early signs ever show themselves.

By the time your A1C is elevated, or your fasting glucose tips into prediabetic range, hyperinsulinemia has already been damaging your biological terrain for a couple of years at minimum.

None of this is speculation on my part: The Whitehall II cohort discovered that that insulin sensitivity declines and compensatory secretion ramps up long before glucose ever moves.

So why isn’t your doctor testing fasting insulin like your life depends on it?

It’s not part of the standard metabolic panel, and the training they received never emphasized the prevention of insulin dysregulation.

As I always say…

This is exactly why finding an optimization-minded physician who actually understands metabolic physiology is non-negotiable if you are serious about your long-term health.

What Fasting Insulin Actually Measures

Some quick background information:

Insulin is the master metabolic hormone secreted by the beta cells of the pancreas in response to rising blood glucose.

Its primary job is to shuttle glucose into cells for energy production, primarily through GLUT4 transporters in muscle and adipose tissue.

When cells stop responding efficiently to that signal (i.e. insulin resistance), the pancreas compensates by secreting MORE insulin to force the same glucose-clearing effect 

Fasting insulin measures how much insulin your pancreas is producing after ~8-12 hours of no food intake.

As you can imagine, higher fasting insulin number tells you your cells are no longer listening properly and your pancreas is working overtime to compensate.

This is the early warning signal your standard metabolic panel and your doctor are completely missing.

The Problem With Relying on Glucose and A1C Alone

The late Dr. Joseph Kraft, a pathologist who spent decades running insulin assays alongside glucose tolerance testing, argued that measuring glucose alone is like checking the smoke alarm battery instead of looking for the fire.

Across thousands of patients he documented, the majority of individuals with perfectly normal glucose had profoundly abnormal insulin responses (a pattern he named “diabetes in situ”)

That work was largely ignored by mainstream endocrinology when it was published, which should tell you everything about how this system prioritizes information.

But back to the discussion at hand…

Hemoglobin A1C measures average blood glucose over roughly 90 days and it is useful, but it is a lagging indicator.

In other words, by the time A1C is clearly rising, insulin has already been elevated for years.

To use another analogy that complements what Dr. Kraft was shouting from the rooftops:

Looking at glucose and A1C alone to assess metabolic health is like checking your rearview mirror to navigate the traffic in front of you.

What “Normal” Really Means (And Why It’s a Lie)

Most commercial labs report fasting insulin as “normal” at 24-25 uIU/mL.

This reference range is built by taking averages from a population that is already metabolically dysfunctional.

When you look at genuinely lean people who are physically active and metabolically healthy people, their “range” for optimal fasting insulin sits between 2-6 uIU/mL.

But regardless of what your glucose or A1C says, anything above 10 uIU/mL for fasting insulin should be highly concerning.

The clinical tool to pair with fasting insulin is HOMA-IR (the Homeostatic Model Assessment of Insulin Resistance) which Matthews and colleagues introduced in Diabetologia back in 1985.

The calculation is simple:

HOMA-IR = (Fasting Insulin uIU/mL x Fasting Glucose mg/dL) / 405

A HOMA-IR below 1.0 is optimal, while anything above 2.0 indicates meaningful insulin resistance.

Once you’re past 2.9, you are now metabolically dysregulated and possess a significantly elevated risk for cardiovascular disease.

What You’re Told What’s Actually True

The Hormone Connection You Can’t Ignore

This is where it gets personal for me and for every man in our community who is serious about optimizing their hormonal health…

Chronically elevated insulin has a direct and mechanistic impact on testosterone production.

Insulin receptors sit on the Leydig cells of the testes (which are responsible for testosterone synthesis) and insulin directly regulates steroidogenesis inside them.

Acute insulin signaling also plays a supportive role there, but chronic hyperinsulinemia does the opposite.

At a certain point, rising insulin resistance tracks with a measurable decrease in Leydig cell testosterone secretion.

Excess insulin also suppresses sex hormone-binding globulin by inhibiting SHBG production directly in the liver.

That is why low SHBG is often the first fingerprint of hyperinsulinemia showing up on a hormone panel, and it’s also why the SHBG and insulin relationship deserves far more attention than it gets.

This in turn means distorted hormone readings, which then leads to accelerated testosterone decline.

Beyond testosterone, elevated insulin:

  • Drives systemic pro-inflammatory cytokine production
  • Accelerates advanced glycation end-product (AGE) formation
  • Promotes fat storage particularly in the visceral depot
  • Impairs mitochondrial efficiency.

SPECIAL NOTE FOR WOMEN: 

Elevated insulin is tied directly to polycystic ovarian syndrome, and hyperinsulinemia plays a central pathogenic role in PCOS rather than a coincidental one.

How to Get Tested and What to Do With the Results

Getting a fasting insulin test is straightforward:

Fast for at least 8 to 12 hours with no food or caloric beverages, then request the test from either your optimization-minded physician or directly through a direct-to-consumer lab.

Ask for the following markers to be measured :

  • Fasting insulin
  • Fasting glucose
  • HOMA-IR calculated from those two values
  • Optionally fasting triglycerides and HDL as additional insulin resistance proxies

Once you have your numbers, the intervention hierarchy looks like this:

  • Nutrition – cutting refined carbohydrate and ultra-processed food is the single most powerful lever, and the framework I use is laid out in my insulin controlled diet
  • Resistance training – skeletal muscle is the primary site of glucose disposal and more muscle means greater insulin sensitivity
  • Zone 2 cardio – improves mitochondrial density and insulin receptor sensitivity directly
  • Sleep optimization – one night of partial sleep deprivation measurably induces insulin resistance the very next day
  • Strategic fasting protocols – reducing insulin secretion frequency allows receptor sensitivity to recover, which I break down in my guide to optimizing intermittent fasting

Do NOT let anyone convince you a prescription medication is the first-line answer here.

Lifestyle-driven improvements in fasting insulin are documented powerful, free of pharmaceutical side effects… and truthfully, FREE to start implementing.

However, neither one is a substitute for fixing the inputs that broke your insulin sensitivity in the first place.

Safety and Contraindications

Understanding your fasting insulin level carries no physical risk.

The risk comes from NOT knowing it and letting insulin dysregulation run unchecked for years.

A few important considerations before I wrap things up:

  • Individuals with type 1 diabetes operate under fundamentally different physiology, and this discussion does not apply to absolute insulin deficiency
  • If you are already on insulin and/or glucose-lowering medication, any dietary or lifestyle change that rapidly lowers insulin requires coordination with your prescribing physician to avoid dangerous hypoglycemia
  • Fasting protocols are not appropriate for pregnant women, anyone with a history of disordered eating, or anyone with significant adrenal dysfunction without proper clinical support
  • A single reading is informative, but trending your numbers over time alongside clinical context gives you the full picture

Take Control of Your Metabolic Future

The most important metabolic biomarker available to you is one your doctor almost certainly is not ordering.

Fasting insulin is the early warning system your physiology has in place.

If you wait until your A1C flags and your doctor finally delivers the bad news about your newfound prediabetic status, you have already lost years of preventive opportunity.

I can’t tell you how many men I’ve seen — even those who were lean — that thought they were healthy because their standard bloodwork looked fine…

… only to discover years later that elevated insulin had been quietly destroying their hormonal environment and inflaming their vasculature the entire time. 

The moral of the story here?

Get your fasting insulin tested and calculate your HOMA-IR ASAP!
