Imagine a woman who starts using testosterone pellets for the very first time.

By week two , she already feels incredible.

Her energy is back and her brain is sharp.

And best of all, her libido is the strongest it’s been in a decade.

But things go south from there…

At week six, her skin starts breaking out along her jawline.

By week ten, she notices dark hairs on her chin and a mood that snaps faster than it used to.

She calls her clinic, and receives the awful news nobody told her prior to insertion:

The pellet is already under her skin and there’s nothing to do but wait three more months for it to wear off.

When it comes to testosterone pellets for women, I AM constantly hearing some version of this story.

Testosterone pellets have become one of the most popular ways women restore their testosterone.

And seeing how it’s the easiest way to get testosterone into any woman’s hormone optimization protocol, I can see why.

But popularity doesn’t mean women understand what they’re signing up for, nor does it represent the most effective method available to women.

Pellets are the delivery method I recommend the LEAST, and I AM going to show you exactly how they work so you can see why I do not recommend their use at all.

Quick Takeaways

  • Testosterone is not a “male hormone” as women need it as much as men do for brain function, muscle, bone density, mood, and libido.
  • Pellets release testosterone over three to five months, but dose can’t be adjusted once they’re in (unless you’re ok with cutting them back out).
  • Pellet doses for women are frequently miscalibrated, and over-dosing is the most common mistake I see.
  • For most women, a compounded and cycle-timed transdermal cream gives far more control over their individual dose… which is the exact approach Monica and I lay out in The TOT Bible.

What Testosterone Pellets Actually Are

Testosterone pellets are small and compressed cylinders of bioidentical testosterone inserted under the skin (usually in the hip or upper glute).

They slowly dissolve over three to five months, releasing testosterone into the bloodstream without having to pass through the liver first.

That first-pass bypass is a pharmacological advantage as oral testosterone adversely affects lipid profiles in women… which is why I never recommend the oral route.

The pellet sits under the skin like a small reservoir, which means it behaves different from a daily cream or a weekly injection.

This makes it both its selling point and its biggest problem.

In The Testosterone Optimization Therapy Bible, I call pellets suboptimal for a simple reason: Once implanted, it’s very difficult to control how much testosterone get released.

And if your labs show you need less, the only fix is to cut open the original site of insertion and remove the pellet.

In my newest book Living Leaner Longer Stronger, I lay out the truth once again: While pellets remove the daily compliance issue, they are highly inefficient and not recommended.

Why Most People Misunderstand Female Testosterone

Testosterone deficiency in women is dramatically underdiagnosed because the medical system was never built to look for it.

Standard hormone panels obsess over estrogen and progesterone, all while treating testosterone readings as a rounding error.

Women produce testosterone in their ovaries and adrenal glands throughout the course of their life, and average levels decline with age long before menopause.

To put this into perspective: Roughly half of a woman’s testosterone levels in her early-20s are GONE by her 40s!

Birth control can make matters worse, since combined oral contraception raises SHBG while driving free testosterone production down.

By the time a woman reaches perimenopause, she may have lost half her peak testosterone output… which she won’t even realize as nobody tested her along the way.

What we’re dealing with here is basic steroidogenesis, i.e. the biological pathway by which cholesterol is converted into testosterone, estrogen, and other important steroid hormones.

Once that pathway slows down, women feel it everywhere: Their energy, mood, cognition, sex drive, and so on

Unfortunately, conventional medicine calls it “aging” instead of being brutally honest about what’s going on.

Even the Global Consensus Position Statement from the major menopause and endocrine societies now recognizes testosterone therapy for postmenopausal women with low sexual desire.

Despite this, most gynecologists still aren’t having the conversation that must be had with their female patients.

The Mechanism: How Pellets Work in the Body

Once implanted, the pellet releases testosterone that binds to androgen receptors throughout the female body (brain tissue, bone, muscle, clitoral tissue, etc.).

That receptor binding drives increased protein synthesis and better insulin sensitivity, along with stronger neurotransmitter activity tied to motivation and mood.

Some of that testosterone undergoes aromatization, which is the normal and necessary enzymatic conversion of testosterone into estradiol within both sexes.

The key difference between pellets and other delivery methods is release kinetics (i.e. the pattern by which the hormone enters circulation).

Pellets are often sold as “perfectly steady” delivery methods… except that isn’t really how they behave as levels tend to climb after insertion and then taper off over the following months.

Furthermore, a woman’s body was never designed to run on levels of sex hormones that are flat and unchanging.

As Monica and I explain in The TOT Bible: A woman’s own testosterone naturally peaks around days 14 to 16 of her cycle, estradiol peaks around day 12, and progesterone peaks around day 21.

A pellet can’t follow that rhythm, nor can it be adjusted if the dose turns out to be too high.

That problem is where most of the real-world issues begin.

Dosing Reality: What I’ve Seen Work vs What Gets Prescribed

Women are exquisitely sensitive to testosterone compared to men, and what counts as a “low normal” male dose can push a woman into supraphysiological territory (i.e. levels far above her natural biological range).

Typical female pellet doses fall somewhere around 50-100 mg every 3-5 months, and despite the right dose being entirely individual to the end user, too many clinics start at the top of that dosing range.

I have seen women walk out of med spas with pellet doses that spike their total testosterone into the 300 to 500 ng/dL range… levels belonging to a MALE reference range!

The results are sadly predictable, and could have been easily avoided with a more conservative starting dose:

  • Acne
  • Unwanted hair growth
  • Voice changes
  • Volatile mood swings

The target is the mid-to-upper end of the premenopausal physiological range, and published female testosterone reference ranges for healthy cycling women are the benchmarks you want to be aiming for.

A smarter approach would be the following:

  • Start with a delivery method you can adjust; that way, you can learn what your individual sensitivity is before anything is implanted (if at all)
  • Get your baseline labs first: Total and free testosterone, SHBG, estradiol, progesterone, hematocrit, and the rest
  • Insist on high-sensitivity LC/MS testing, as standard immunoassays are unreliable at female testosterone concentrations
  • If you’re still cycling: Test testosterone on day 14, estradiol on day 12, and progesterone on day 21
  • Retest your labs at 6 to 8 weeks, then every 3 to 6 months while tracking symptoms at the same time
  • Never accept a “standard dose” without seeing your baseline numbers first.

Pellets vs Other Delivery Methods

Every delivery method has tradeoffs, and pellets are not automatically the “best” option for any woman.

Delivery Method Typical Female Dose Release Pattern Reversibility Notes
Compounded transdermal cream 0.5 mg to 2 mg daily Can be cycle-timed to mirror natural rhythm High, adjust daily The approach I teach, and the most precisely titratable
Very low-dose injection Individualized, expert oversight Peaks then declines between doses Moderate, adjust next dose Monica knows women who do excellently on extremely low doses
Pellets Roughly 50 mg to 100 mg every 3 to 5 months Rises after insertion, tapers over months Very low, requires removal Convenient, but the least controllable option
Oral testosterone Not recommended Passes through the liver High Adversely affects lipid profiles

I personally favor delivery methods that allow for rapid course correction.

A compounded cream dosed on a cycle calendar respects the way a woman’s body actually works, and if something feels off, she can change the dose tomorrow instead of waiting for months.

If a woman has already worked with an optimization-minded physician, knows her sensitivity, and yet still chooses pellets for convenience… well that’s her call to make with a clear and conscious mind.

However, starting with pellets BEFORE knowing how you’ll respond to testosterone is typically where I see women get burned.

Myth vs Reality

Let’s kill some of the nonsense circulating around this topic…

Myth: Testosterone will make women “bulky” and/or masculinize them at optimization doses.

Reality: Masculinizing effects come from supraphysiological dosing, which is not the same thing as restoring youthful physiological levels.

Myth: Only menopausal women need testosterone therapy.

Reality: Testosterone decline begins in a woman’s 20s and 30s, long before menopause, and some symptoms can even appear decades earlier.

Myth: Pellets are “set it and forget it,” so you don’t need monitoring.

Reality: Pellets still require lab monitoring, and the frustrating part is that you can’t adjust the dose mid-cycle when your labs run high,

Myth: Pellets are the most natural option because they’re steady.

Reality: A woman’s hormones were never meant to be flat, and a cycle-timed cream mirrors female physiology far better than a pellet can.

Myth: All women need the same dose.

Reality: Individual sensitivity to androgens varies enormously… and that’s exactly why dosing must be personalized to the individual.

Safety, Risks, and Contraindications

Testosterone pellets are NOT risk-free!

Common risks include:

  • Acne and oily skin from excess androgen activity at the pilosebaceous unit
  • Hirsutism (unwanted hair growth) from prolonged supraphysiological exposure
  • Voice deepening, which may not fully reverse if it occurs, remains the strongest argument against chasing ranges designed for men
  • Insertion site infection, bleeding, and/or pellet extrusion are mechanical risks of the procedure itself… and research on testosterone implants shows higher extrusion rates at the hip than the abdomen.

Contraindications and caution areas can include:

  • Active or prior hormone-sensitive cancer
  • Pregnancy and breastfeeding
  • Uncontrolled polycythemia or undiagnosed abnormal uterine bleeding
  • Combining pellets with other androgenic compounds without professional guidance

THE RISK IS UNMONITORED AND POORLY DOSED TESTOSTERONE, NOT TESTOSTERONE ITSELF!

This is a critical distinction that mainstream media conveniently ignores when they’re writing fear-based headlines and hit pieces about hormone therapy.

Female-Specific Considerations

Women are not small men, and their hormone protocols should never be built with that type of faulty thinking.

In The TOT Bible, Monica and I built female hormone optimization on four “B” principles: Bioidentical, biorhythmic, balanced, and bioavailable.

Pellets can check the bioidentical box, but they struggle with being biorhythmic as a pellet can’t rise and fall with a woman’s cycle.

“Balanced” means testosterone is one leg of a triad alongside estradiol and progesterone, which means any concurrent estrogen and progesterone therapy will always changes how a woman responds to hormone replacement therapy.

Monica’s guidelines for every woman are simple: Choose ONE goal, start low, go slow, and listen to your body (because there is no such thing as a “perfect” protocol).

She personally doesn’t do well with transdermal delivery because of acne development, which is yet another reason why individualization matters more than any single “best” method.

Women considering any form testosterone therapy should get a complete baseline panel, and I break down every marker worth running in my guide to longevity lab tests.

Finding the Right Practitioner

Not every hormone provider understands female androgen physiology, and this is where most women are heavily underserved.

Look for a practitioner who practices optimization medicine and is focused on restoring youthful physiological function.

Ask direct questions before working with any physician:

  • What dose are you starting me on, and why?
  • Why pellets instead of a cream I can adjust?
  • How often will you retest, and on which cycle day?
  • What symptoms should trigger a dose change?

If a provider cannot answer those questions with specifics, that is your signal to look elsewhere, and my guide on how to find an optimization-minded physician will set you on the right path.

Testosterone Pellets For Women: The Bottom Line

Testosterone can be a powerful tool for restoring energy, libido, cognitive sharpness, and body composition in women…

… but ONLY when it’s dosed with precision and monitored with discipline.

Pellets are a convenient way to get testosterone into a woman’s body, but convenience becomes a problem when the dose can’t be walked back.
