The standard annual blood panel is NOT designed to optimize the health of men over the age of 35. 

At best, it will rule out catastrophic disease and generate another bill for your insurance company.

Your doctor will look at your results, say “everything checks out,” and send you home without a single conversation about enhancing your health if you fit right on average.

I have spent over 25 years running my own labs, collaborating with some of the most forward-thinking clinicians and researchers on the planet, and building a community of men who refuse to accept “normal” as the standard.

What I am about to share here is information you will not find anywhere else. 

These are the 5 best lab tests for longevity that ACTUALLY tell you what is happening inside your body at a mechanistic level.

And soon enough, you’ll see why your conventional doctor almost certainly will not order them for you (even if you ask nicely).

Why Standard Blood Panels Fail You

The conventional model of medicine is designed from the ground up to be reactive.

Your doctor, first and foremost, has been trained to exclusively look for pathology.

Their job is to catch the values so far outside a population-based reference range that they suggest active disease.

When most doctors tell you “your lab results are normal,” what they’re really saying is you are “normal” compared to a range built from averaging out a large population.

What you don’t get told is this large population is full of sedentary and highly-inflamed individuals. 

Falling inside this range doesn’t mean everything in your body is working as intended.

You’re just simply not sick to the point where your situation is catastrophic.

This tells you absolutely NOTHING!

You have no idea:

  • In which direction you are trending
  • How efficiently your metabolism is functioning
  • How inflamed your vascular system is
  • Whether your hormones are supporting the body you want to live in for the next 40 years

The five tests I AM about to introduce to you, however, are the furthest thing from exotic or experimental.

Each of them are widely validated and clinically available.

Yet remain systematically ignored by mainstream medicine because the system has no financial incentive to prevent disease.

Only to “manage” it over a long-enough time horizon.

Lab Test #1: Fasting Insulin and HOMA-IR

Your doctor measures fasting glucose and HbA1c to check for Type 2 diabetes.

A good start, but with one major flaw:

By the time those markers are significantly elevated, insulin resistance has often been silently destroying your metabolic health for at least a few years. 

And if you don’t know WTF insulin resistance is…

Insulin resistance is a condition where cells become progressively less responsive to insulin signaling, forcing the pancreas to produce increasingly higher amounts of insulin to maintain blood glucose control.

Your fasting insulin, one of the signals I recommend you watch closely, is the early warning signal.

A fasting insulin value above 8-10 uIU/mL is already a red flag in the optimization world, even if your glucose readings look perfectly “normal.”

HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is calculated using both fasting glucose and fasting insulin, providing you with a composite picture of insulin sensitivity that neither marker alone provides.

Research consistently shows elevated fasting insulin and HOMA-IR predict the onset of several diseases (cardiovascular disease, neurodegenerative decline, obesity, accelerated aging, etc.) long before glucose dysregulation becomes apparent on a standard panel.

Conventional clinical cutoffs for HOMA-IR sit around 2.0 to 2.5, which is where the system starts calling it insulin resistance.

But again, this is a disease threshold that does not necessarily represent an “optimization” target.

My target for HOMA-IR: Below 1.5, and below 1.0 is optimal.

Yes, this is a tighter standard than mainstream medicine uses because metabolic risk starts climbing well before you hit the conventional cutoff.

If you are not measuring fasting insulin, you are flying blind on one of the most consequential metabolic pathways in your entire physiology.

And my insulin controlled diet framework exists for the sole purpose of addressing this.

Lab Test #2: ApoB

Your standard lipid panel measures total cholesterol, LDL-C, HDL-C, and triglycerides.

Here is what this data DOES NOT tell you: How many atherogenic lipoprotein particles are actively circulating in your bloodstream.

Apolipoprotein B (ApoB) is a structural protein found on every atherogenic lipoprotein particle (including LDL, VLDL, IDL, and Lp(a)).

Each particle carries exactly one ApoB molecule, which makes ApoB a direct count of your total atherogenic particle burden.

This matters because two people can have identical LDL-C values while one has twice as many particles… and therefore a dramatically higher risk of  cardiovascular disease.

To make this distinction even more clear:

  • LDL-C measures cholesterol mass
  • ApoB measures particle number
  • Particle number is what drives atherosclerotic plaque formation through endothelial penetration and oxidation

Research led by Dr. Allan Sniderman repeatedly demonstrates ApoB is a superior predictor of cardiovascular events compared to LDL-C.

Furthermore, a 2025 systematic review of discordance analyses covering roughly 593,000 participants found ApoB outperformed LDL-C in 9 out of 9 studies examined.

And the European Atherosclerosis Society and European Society of Cardiology guidelines already recognize ApoB as the better risk marker.

Optimal ApoB for longevity-focused individuals: Below 80 mg/dL, and below 70 mg/dL if you carry additional cardiovascular risk.

If your cardiologist is NOT measuring ApoB, find an optimization-minded physician who understands the importance of modern cardiovascular risk stratification.

Lab Test #3: High-Sensitivity C-Reactive Protein (hs-CRP)

Inflammation is the silent driver of nearly every chronic disease your doctor is trying to prevent.

The standard CRP test is a blunt instrument designed to detect acute infection or severe inflammatory events.

Not bad, but far from complete or comprehensive.

High-sensitivity C-reactive protein (hs-CRP) is a precision measurement that detects low-grade systemic inflammation at levels the standard CRP assay simply cannot detect.

Chronic low-grade inflammation silently does the following, and much more over many years and decades:

  • Damages arterial walls
  • Accelerates biological aging
  • Disrupts hormonal signaling
  • Drives neurodegenerative pathways over years and decades

To illustrate what you’re missing out by ignoring hs-CRP, the landmark JUPITER trial enrolled nearly 18,000 apparently healthy people who had LDL-C below 130 mg/dL but hs-CRP at or above 2.0 mg/L.

When they looked at them further, this particular cohort carried enough residual cardiovascular risk that intervention produced a 44% relative risk reduction in major events.

Pay close attention: These were people whose cholesterol panels would have gotten them waved out of a conventional doctor’s office!

I have seen men walk into conversations with me carrying “perfect” cholesterol panels.

Meanwhile, their hs-CRP is sitting at 4.0 or higher, indicating their vascular system is under constant inflammatory assault.

Your new hs-CRP target: Below 1.0 mg/L, and below 0.5 mg/L is what I consider truly optimized.

Anything above 3.0 mg/L that is not explained by acute illness deserves an immediate investigation into the root cause.

The root cause(s) in question is almost always diet, gut dysfunction, metabolic dysfunction, and/or hormonal disruption.

Lab Test #4: Homocysteine

This one is particularly frustrating, because most doctors still do not test for it routinely.

Despite the fact homocysteine has been linked to cardiovascular disease, cognitive decline, and mortality risk in the research literature for decades.

Homocysteine is a sulfur-containing amino acid produced during the metabolism of methionine, a process that is tightly regulated by B vitamins (specifically folate, B6, and B12).

Elevated homocysteine is a marker of impaired methylation, one of the most critical biochemical processes in the human body.

FYI — Methylation governs DNA repair, neurotransmitter synthesis, detoxification, and gene expression through epigenetic regulation.

When methylation is impaired, homocysteine accumulates and directly damages vascular endothelium.

Along with the promotion of oxidative stress and cholesterol-independent plaque formation.

Prospective research in healthy older adults finds people with homocysteine at 14 umol/L or higher carried roughly double the risk of Alzheimer’s disease after adjusting for known risk factors.

Many people carry MTHFR polymorphisms — genetic variants that impair the enzyme responsible for converting folate into its active form — making them structurally prone to elevated homocysteine regardless of diet.

Now, here is where I AM going to interject and provide some further nuance to this conversation… 

The association between elevated homocysteine and disease risk is rock solid.

What is NOT settled is whether simply driving the number down with B vitamins reduces your actual event risk.

Large randomized trials of B-vitamin supplementation have reliably lowered homocysteine, while producing mixed and often disappointing results on cardiovascular endpoints and cognitive decline.

The trials that DID show benefit, like VITACOG, tended to target high-risk people with existing mild cognitive impairment and poor baseline B vitamin status.

This should tell you something important about who actually responds.

So why do I still test it in spite of this contradiction?

Because homocysteine is a window into methylation function, and methylation has its hands deep in various genetic and biological cascades across your entire physiology.

A number above 12 tells you something is wrong upstream.

Therefore, the appropriate response is is to investigate WHY instead of chugging methylfolate and declare victory on the lab value.

Your new homocysteine target: Below 9 umol/L, with below 7 umol/L optimal.

Test it and treat the root cause, rather than the number on the blood panel itself.

Lab Test #5: A Full Sex Hormone Panel

This is the one I feel most personally connected to as hormone optimization has been the foundation of my own transformation and the work I have dedicated my career to.

Your doctor will check total testosterone if you push them hard enough.

Yet that single number tells you almost nothing actionable.

A truly informative hormone panel will include all of the following at minimum:

  • Total testosterone
  • Free testosterone (bioavailable, unbound fraction)
  • SHBG (Sex Hormone-Binding Globulin), the protein that binds testosterone and renders it inactive
  • Estradiol (sensitive assay, not standard)
  • DHEA-S (dehydroepiandrosterone sulfate, the precursor hormone reflecting adrenal reserve)
  • Pregnenolone (the master precursor hormone at the top of the steroid synthesis cascade)
  • LH and FSH (to assess pituitary-gonadal axis function)
  • Prolactin (elevated levels suppress testosterone and libido)

Here is the most common observation I end up making to this day:

Men with total testosterone in the “normal” range of 450-500 ng/dL who feel absolutely terrible because their SHBG is so high that their free testosterone is severely depleted.

Free testosterone is what your tissues actually use.

Which means total testosterone without SHBG context is nearly meaningless for optimization purposes.

Estradiol in men is equally misunderstood.

  • Too low and you lose bone density, libido, and cognitive function
  • Too high and you drive fat storage, emotional dysregulation, and cardiovascular risk

The balance matters enormously, and optimal estradiol levels for men on testosterone replacement therapy (TRT) are a critical part of this picture.

The optimization-minded physician evaluates all of these markers together as a system if they’re doing their job properly.

A Special Note for Women

Hormone optimization is not just exclusive to the dudes.

Monica has been vocal about the fact women are arguably even more underserved by conventional medicine when it comes to hormonal assessment.

Women require a full panel including estradiol, progesterone, testosterone, DHEA-S, and SHBG evaluated in the context of their cycle phase or menopausal status.

The same principle applies as with the men: Total hormones without free fractions and binding proteins tell an incomplete and potentially misleading story.

Myth vs. Reality: What These Tests Actually Mean

Conventional Assumption Optimization Reality
Normal glucose means no insulin problem Fasting insulin can be dangerously elevated with normal glucose

How to Get These Tests Ordered

DO NOT simply ask your primary care doctor for these tests and accept a refusal without pushing back.

If you are being dismissed, you have two options.

First, find an optimization-minded physician (alternatively a functional medicine doctor, a hormone specialist, or a longevity-focused clinician) who understands what you are asking for and why.

Second, use direct-to-consumer lab services that allow you to order your own bloodwork without a physician’s order in most states.

Owning your own health data is the first act of health sovereignty.

NEVER outsource your understanding of your own labs entirely to a doctor who has eight minutes with you and a disease-management framework.

Learn what your numbers mean and track the trends over time. 

And if you’re fortunate enough, build a relationship with an optimization-minded clinician who will work WITH you and FOR you!

The sick-care system is designed to manage disease endpoints and operate within insurance reimbursement structures that have nothing to do with your longevity or quality of life.

Whereas the five tests I covered today are evidence-backed and clinically validated markers that leading researchers and optimization physicians have been using for years.

  • Fasting insulin and HOMA-IR reveal metabolic dysfunction a decade before diabetes is diagnosed.
  • ApoB tells you what your real cardiovascular particle burden is.
  • hs-CRP exposes silent vascular inflammation before it becomes a cardiac event.
  • Homocysteine shows whether your methylation system is functioning and protecting your brain and arteries.
  • A full sex hormone panel reveals whether your entire hormonal ecosystem is actually supporting the body and mind you want to operate.

You now know what to ask for.
