A Note on Language Before We Start
In my line of work, Monica and I use the term hormone optimization therapy (HOT) for women rather than hormone replacement therapy (HRT).
“Replacement” implies topping a tank back up to some mediocre baseline.
While “optimization” means restoring the hormonal environment that lets a woman thrive.
This distinction serves as the basis of everything you’re about to read.
Why Women Need Testosterone: The Physiology They Never Taught You
Most people associate testosterone exclusively with men.
Unfortunately, this half-truth is what costs millions of women their quality of life.
Women produce testosterone in the ovaries and the adrenal glands.
Androgens, the class of hormones including testosterone and DHEA (dehydroepiandrosterone), are present in women at measurable and physiologically meaningful concentrations throughout their entire lives.
Androgen receptors (ARs) are distributed throughout the female body, including the brain, bone, cardiovascular tissue, skeletal muscle, vaginal tissue, and adipose tissue.
Production of the androgen hormones in women peak in the mid-20s and decline steadily from there.
The main problem is while women age (particularly as they approach perimenopause and menopause) their testosterone production can drop substantially… oftentimes faster than estrogen.
When testosterone drops, those receptors are starved and the downstream biological consequences are severe.
So right away, we can see the problem is NOT related to women having lower testosterone levels compared to men.
The Real Benefits of Testosterone for Women
Libido and Sexual Function
Believe it or not, testosterone is a primary hormonal driver of sexual desire in women.
Hypoactive sexual desire disorder (HSDD) affects roughly 10 percent of women, with higher rates after menopause being observed.
Multiple randomized controlled trials have demonstrated testosterone therapy meaningfully restores sexual desire, arousal, orgasm, and satisfaction in postmenopausal women.
As a matter of fact, eleven separate international medical societies have formally endorsed HOT/HRT for this indication in their 2019 Global Consensus Position Statement.
The establishment’s own consensus body reviewed the randomized trial data and concluded testosterone clearly works… but your doctor won’t offer it to you!
Put another way…
The androgen receptor signaling pathway in the hypothalamus directly modulates dopaminergic reward circuits tied to sexual motivation.
When testosterone is restored to optimal physiological range, those circuits light back up.
Cognitive Function and Mood
Brain fog in women is an epidemic being treated with antidepressants at alarming rates.
But testosterone has neuroprotective properties and directly influences serotonergic and dopaminergic neurotransmitter systems.
On top of that, androgen receptors are densely expressed in the regions that govern memory, executive function, and emotional regulation (hippocampus, prefrontal cortex, amygdala, etc.)
Women with low testosterone frequently report the following hormonal deficiency symptoms:
- Difficulty concentrating
- Memory lapses
- Emotional flatness or irritability
- Loss of competitive drive and motivation
The randomized trial data for testosterone in women is strongest for sexual function, and thinner for outcomes such as mood and cognition.
But the limited evidence available for the latter two is not the same thing as negative evidence.
And frankly, I have watched too many women get their minds back on properly managed protocols to pretend something isn’t happening under the hood.
Androgen receptors do not stop existing in the hippocampus just because the right clinical trial has not been funded yet.
Bone Density and Musculoskeletal Health
Osteoporosis is framed as an estrogen problem, but that is only half the story
Testosterone drives periosteal bone formation, which is the process that builds bone width and cortical density.
Estrogen, on the other hand, primarily prevents bone resorption.
You require BOTH hormones working in concert to maintain genuinely strong and fracture-resistant bone tissue.
Additionally, testosterone activates androgen receptors in skeletal muscle to drive muscle protein synthesis (i.e. the same fundamental mechanism that makes testosterone anabolic in men)
Women who optimize testosterone can build and maintain lean muscle mass with far greater efficiency.
Long-time readers know staying lean with an appreciable amount of skeletal muscle tissue is one of the most powerful longevity strategies available to any human being.
Cardiovascular and Metabolic Protection
The metabolic effects of testosterone in women are chronically underreported.
Testosterone supports insulin sensitivity, favorable lipid partitioning, and the reduced accumulation visceral adipose tissue.
Visceral fat is the metabolic villain driving systemic inflammation and insulin resistance, along with elevated cardiovascular risk.
Not to mention women with androgen deficiency have measurably worse metabolic profiles.
However, the long-term cardiovascular outcome trials in women simply have not been done, so anyone promising you cardiac protection is getting ahead of the data.
The main takeaway is this:
Restoring physiological testosterone restructures a woman’s metabolic terrain in a favorable direction, and metabolic terrain is what drives long-term risk.
Energy, Vitality, and Resilience
This one is harder to quantify in a clinical trial, but I have seen it play out repeatedly in real life.
Women on optimized protocols describe a return to their “real selves”:
- More energy
- More drive
- A sense of agency and vitality that fatigue and hormonal decline have quietly stolen from them

What Low Testosterone Actually Looks Like in Women
Symptoms of androgen deficiency in women are frequently misdiagnosed as “normal aging.”
Things like depression, chronic fatigue syndrome, autoimmune disorders, and thyroid dysfunction get swept under the rug.
Here is what I want you to watch for:
- Persistent fatigue that does not resolve with sleep
- Loss of sex drive or inability to achieve orgasm
- Muscle weakness and difficulty building or maintaining lean mass
- Mood instability, anxiety, or emotional blunting
- Cognitive decline, brain fog, and memory issues
- Thinning hair and skin quality changes
- Reduced motivation and competitive drive
If you recognize several of these… you are not crazy, and you are not simply “getting older.”
You may be hormonally deficient and getting zero support from a system designed for symptom management rather than root cause detection.
The Four Principles That Should Govern Any Female Protocol
This is the part almost every article on this topic skips, which is a real shame because this part is what actually determines whether a woman thrives or suffers.
Monica and I have written extensively about four principles any legitimate female hormone protocol must honor…
Bioidentical: The hormones used must be molecularly identical to what a woman’s body produces.
Conjugated horse estrogen, ethinyl estradiol, and synthetic progestins are not human hormones.
And the Women’s Health Initiative showed us what happens when the industry pretends otherwise.
TL;DR = Synthetic sucks.
Biorhythmic: A young woman’s hormones move in rhythmic waves across a 28-day cycle, with an estradiol peak around day 12 and a testosterone peak around days 14-16.
Those rhythms exist for a reason.
Therefore, flat and unchanging daily doses ignore the entire design of female physiology.
Balanced. Testosterone does not work in isolation.
Estradiol and progesterone have to be in proper relationship to testosterone (and to each other).
Bioavailable. Transdermal delivery over pills.
Oral hormones get hit by first-pass liver metabolism, which creates harmful byproducts and drives up SHBG, thereby binding the very testosterone you are attempting to raise.
Finally, please understand that women are considerably harder to optimize hormonally than men.
A woman’s hormonal situation is far more dynamic, and optimization in this regard genuinely requires a physician who has successfully worked with hundreds of women.
There is also a legitimate debate to be had for the merits of cyclic and static dosing approaches, but the honest answer is that it comes down to the individual woman.
Safety, Risks, and What to Watch For
Testosterone therapy in women is safe WHEN IT IS DONE CORRECTLY!
Key safety parameters to watch out for include:
Starting low
Female testosterone doses are dramatically lower than male doses, typically in the range of roughly 2.5 to 10mg per day for transdermal preparations (depending on delivery method and individual response).
That is about 1/10th of a male dose.
This is a milligram-range conversation, and a woman who lands on a male dosing chart will end up becoming masculinized.
Delivery methods matter enormously
Compounded transdermal creams are the preferred route.
Pellets and standard injectable protocols routinely push women into supraphysiological territory, which is exactly where the masculinizing side effects live.
The international consensus statement specifically advises against them for this reason.
Some women do well on very low-dose injections, but this route requires the guidance of an experienced clinician.
Monitor labs consistently, and time them to your cycle
Free testosterone, total testosterone, SHBG, hematocrit, and relevant metabolic markers should be measured at baseline and on a regular schedule.
If you are still cycling, the day you draw your blood tests matters enormously.
Test testosterone around day 14, estradiol around day 12, progesterone around day 21.
A lab result without a cycle-day reference point is close to meaningless.
Watch for androgenic side effects
Acne, oily skin, increased body hair, and clitoral sensitivity changes can occur if doses are excessive.
Fortunately, these are usually dose-dependent and reversible if caught early.
However, voice deepening is not reliably reversible, which is precisely why overshooting the physiological range is unacceptable.
Do not use testosterone in active hormone-sensitive malignancies without the explicit guidance of a specialist
The randomized trials excluded women with a prior breast cancer diagnosis, so caution here is warranted rather than optional.
Find an optimization-minded physician who understands female androgen physiology.
DO NOT work with a a conventional OB-GYN who has never been trained in this area, and who defaults to what is FDA-approved over what the science actually supports.
Your health deserves far better.
How to Actually Get Started
Step one is getting the right blood tests.
At minimum, insist on the following:
- Total testosterone
- Free testosterone
- SHBG
- Estradiol
- Progesterone
- DHEA-S
- Thyroid panel (TSH, Free T3, Free T4)
- Complete metabolic panel
Use high-sensitivity (LC/MS) lab methodology wherever possible, especially for testosterone (since standard assays are calibrated for male ranges and perform poorly at female concentrations).
Step two is finding a clinician who understands female hormone optimization.
Run away from anyone who tells you your testosterone is “normal” because it falls within a reference range built on population averages.
Step three is patience and consistent monitoring.
Hormonal optimization is an ongoing and data-driven process that requires regular re-evaluation.
