And as someone who has been on both forms of therapy for 25 years, and I can tell you the single most important decision in your entire hormone optimization journey will be the physician you choose to work with.

This decision matters more than anything else you’ll do because the wrong doctor will keep you sick and suboptimal for years to come.

You MUST learn how to find a doctor for peptide therapy and hormone optimization who knows how to do things properly from start to finish.

Why Your Primary Care Doctor Is the Wrong Person for This Job

Mainstream medicine — what I colloquially refer to as the “sick-care system” — was built to treat disease by medicating symptoms.

Optimizing a healthy human being is NOT top of mind for your average physician.

So when you walk into a primary care office at 42 years old with brain fog, a disappearing libido, stubborn belly fat, and total testosterone levels at 380 ng/dL, you will almost certainly hear the two most dangerous words in medicine:

“You’re normal.”

Normal is a statistical range built from an increasingly sick and overweight population that has fallen victim to endocrine-disrupting chemicals.

Normal is the furthest thing from OPTIMAL, and any physician who can’t articulate or recognize the difference has already disqualified themselves from managing your hormones.

The doctor you’re looking for belongs to an entirely different breed of healthcare providers:

A progressive-minded physician, ideally with more than five years of direct experience treating men and women with hormone therapy.

Bonus points if they’ve used hormone therapy successfully on themselves.

If someone is giving you advice on optimizing your health, their own physical condition is a reliable indicator of their credibility.

Can a Doctor Prescribe Peptides?

Any licensed physician in the United States can legally prescribe peptides that are available through FDA-registered compounding pharmacies and/or as approved medications.

For example, Semaglutide and Tirzepatide are FDA-approved GLP-1 peptides your doctor can prescribe today.

Many other therapeutic peptides are prescribed off-label or compounded through licensed 503A and 503B pharmacies under a physician’s authority.

The real question is who can prescribe peptides COMPETENTLY.

And the list of qualified candidates is dramatically shorter.

The overwhelming majority of physicians have received zero education on therapeutic peptides in medical school.

Ask your average internist about BPC-157 or Tesamorelin and watch their eyes glaze over.

Or worse, watch them dismiss an entire category of compounds they’ve never studied.

In practical terms, you’re looking at a few categories of prescribers:

  • Hormone optimization and age-management physicians — MDs and DOs who have built their entire practice around prioritizing health optimization over disease management.
  • Progressive endocrinologists and urologists — rare, but they do exist. The specialty matters far less than the individual’s experience and philosophy.
  • Nurse practitioners and physician assistants working under optimization-focused practices — in most states, they can prescribe testosterone (a Schedule III controlled substance) and therapeutic peptides.
  • Telehealth optimization clinics — physician networks that specialize in hormone and peptide therapy delivered remotely, with blood work drawn locally.

Notice who’s NOT on that list: The generalist who sees 40 patients a day and has never managed a single person on testosterone optimization therapy over an extended period of time.

Credentials is what gets a prescriber in the door.

Experience treating hundreds of patients on these exact therapies is what actually protects you.

The 4 Red Flags That Mean You Should Walk Away Immediately

You’re effectively disrupting your body’s natural hormone production when you begin hormone therapy, and careless use of exogenous hormones carries serious yet well-documented risks.

Which is why these four red flags are non-negotiable:

  1. A god complex. The “White Lab-Coat God Complexed Dictator” (as I called them in The TOT Bible) who won’t explain their reasoning. You need someone who will be a willing partner in the improvement of your health.
  2. Zero prior experience prescribing hormone therapy. You are NOT volunteering to be someone’s first attempt or lab experiment.
  3. Recommending salivary testing for testosterone. This alone tells you exactly how current their knowledge base is, and the answer in this context is “decades out of date.”
  4. Treating your hormonal health as an “I’ll try it and see what happens” experiment. Guesswork is for hobbies, never for your endocrine system.

Just ONE of these is enough to end the relationship on the spot.

The 4 Questions That Expose an Unqualified Prescriber

Before you commit a single dollar to any hormone optimization doctor, ask the following questions:

  1. “How many patients have you treated long-term on this therapy and for how long?” Starting patients is easy, but managing them successfully for years is the actual skill you’re looking for.
  2. “What’s your monitoring schedule, and which labs do you test for specifically?” A qualified physician will test for total and free testosterone, estradiol (sensitive assay), hemoglobin and hematocrit, and a complete blood count without hesitation. A blank stare here is your exit cue.
  3. “What’s your actual stance on aromatase inhibitors?” There is no single “correct” answer, but a thoughtful and experience-based position separates real practitioners from protocol-followers reading off a script.
  4. “What’s your experience with women’s hormone optimization?” Especially critical if you’re a woman — cycle timing changes everything, and a hormone panel without a noted cycle day attached is close to meaningless.

A qualified physician will welcome these questions, while an unqualified one will bristle at them.

What a Real Hormone Optimization Doctor Monitors

The right physician treats bloodwork as a living document requiring constant updates.

At minimum, expect the following:

  • Total and free testosterone drawn 2-3 times per year
  • Hemoglobin and hematocrit monitored to stay below 20 g/dL and 52-54% respectively
  • Acomplete blood count twice in your first year
  • Estradiol tested via high-sensitivity LC/MS methodology

For women, labs must be timed to the cycle: Estradiol on day 12, testosterone on day 14, progesterone on day 21.

And through it all, one principle governs everything: Symptoms always outrank the number on the page.

A doctor who treats the lab value instead of the human being in front of them has missed the entire point of optimization.
