Two drugs that’ll take the world by storm for the current decade will be – Semaglutide and Tirzepatide.

You’ve probably heard of them by their brand names – Wegovy, Ozempic (Semaglutide), and Mounjaro, Zepbound (Tirzepatide).

These medications are transforming how we approach weight loss, offering hope to millions struggling with obesity.

With Semaglutide alone projected to make up $7 billion of the projected $30 billion in global sales of obesity drugs by 2030, you can see why healthcare providers are paying attention.

While these drugs might seem similar at first, they have some key differences that you should be aware of.

What Is Semaglutide?

Semaglutide is a “GLP-1 receptor agonist” (a hormone-like medication that mimics your body’s natural fullness signals). It is one of the best peptides for weight loss out there.

Think of it as a synthetic version of a hormone your body naturally produces when you eat. This hormone, GLP-1, helps control your blood sugar and tells your brain when you’re full.

The FDA has approved Semaglutide under different brand names for different uses:

  • Ozempic and Rybelsus: For type 2 diabetes
  • Wegovy: Specifically for weight loss

Here’s what makes Semaglutide unique: it’s 94% identical to your body’s natural GLP-1 hormone, but scientists have modified it to last much longer in your system.

While your natural GLP-1 breaks down quickly, Semaglutide sticks around for a week, so you only need one dose every seven days.

But how does it work in your body? Let’s break it down:

  • Blood Sugar Control:
    • Tells your pancreas to produce more insulin when your blood sugar is high
    • Reduces the amount of glucagon (a hormone that raises blood sugar)
    • Clinical trials showed significant improvements in blood sugar levels within just 12 weeks
  • Weight Loss:
    • Slows down how quickly food moves through your stomach
    • Sends signals to your brain that you’re full
    • Reduces your appetite naturally

The STEP clinical trials (a major research program) showed impressive results.ย 

Participants receiving 2.4mg once-weekly Semaglutide achieved a mean weight loss of 14.9% compared to only 2.4% in the placebo group (STEP 1).

Combined with intensive behavioral therapy (STEP 3), participants lost an average of 16.8 kg compared to 6.2 kg in the placebo group.

86.4% of participants on Semaglutide achieved a weight loss of more than 5%, 69.1% achieved a weight loss of more than 10%, and 50.5% achieved a weight loss of more than 15%.

The STEP 4 trial showed the importance of continued treatment. When participants switched to placebo after 20 weeks, they regained 6.9% of their body weight over the next 48 weeks, while those who continued Semaglutide maintained their weight loss.

In 2023, the FDA approved Wegovy (the weight loss version) for people as young as 12. This makes it one of the few effective weight-loss medications for adolescents.

Semaglutide Dosage

Semaglutide is available in two forms: an injection under the skin (subcutaneous) or tablets taken by mouth.

The injection is more commonly used, and your exact dosage will depend on whether you’re taking it for diabetes or weight loss.

Let’s break down the dosing schedules for each use:

For Type 2 Diabetes (Ozempic)

If you’re using Semaglutide for diabetes management, here’s how the dosing typically works:

  • You’ll start with 0.25mg once a week for your first 4 weeks
  • Then bump up to 0.50mg weekly
  • If needed, your doctor might increase your dose up to 2mg per week for better blood sugar control

Pro tips:

  • Pick the same day each week for your injection
  • If you miss a dose, take it within 5 days, then continue your regular schedule

For Weight Loss (Wegovy)

The weight management dose schedule is a bit more detailed:

  • First 4 weeks: Start with 0.25mg weekly
  • Weeks 5-8: Increase to 0.5mg weekly
  • Weeks 9-12: Move up to 1mg weekly
  • Weeks 13-16: Increase to 1.7mg weekly
  • Week 17 onwards: Reach the full dose of 2.4mg weekly

If you can’t tolerate at least 1.7mg weekly, your doctor will likely suggest stopping treatment.

Taking Semaglutide as Pills (Rybelsus)

The newest addition to the Semaglutide family is the oral version called Rybelsus.

Here’s how to take it:

  • Begin with one 3mg pill daily for 30 days
  • Then increase to 7mg daily
  • If needed, your doctor might bump you up to 14mg daily after another 30 days

Some important tips for taking the pills:

  • Take them whole with just a sip of water
  • Wait at least 30 minutes before eating or drinking anything
  • If you miss a dose, skip it – don’t try to make it up

(Note: The injection and pill versions work differently in your body, so there’s no direct way to switch between them.)

Semaglutide Cost

Without insurance, Semaglutide can cost around $1,300-1,500 per month.

According to 2022 research, a 68-week treatment course costs approximately $22,878.ย 

The annual cost is about $17,495, roughly $336 per week.

In terms of value per result, you’ll spend around $1,845 for every 1% of weight loss achieved, though this can range from $1,707 to $1,989, depending on your response to the medication.

The good news is that many insurance plans now cover it, particularly for diabetes treatment.

Side Effects of Semaglutide

Over multiple large-scale clinical trials (STEP, SUSTAIN, and PIONEER), researchers have built a clear picture of how this medication works and what side effects you might experience.

The largest research comes from the STEP-1 trial, which included 1,306 participants taking Semaglutide and 655 receiving a placebo.ย 

The most common side effects were digestive system issues (gastrointestinal or GI side effects). 74.2% of people taking Semaglutide (969 participants) experienced some GI effects, compared to 47.9% (314 participants) in the placebo group.

Here’s the breakdown of specific side effects in the Semaglutide group:

  • Nausea: 577 people (44.2%)
  • Diarrhea: 412 people (31.5%)
  • Vomiting: 324 people (24.8%)
  • Constipation: 306 people (23.4%)
  • Nasopharyngitis (cold-like symptoms): 281 people (21.5%)
  • Dyspepsia (indigestion): 135 people (10.3%)
  • Abdominal pain: 130 people (10.0%)

The research also found that 8.7-15.2% of people in both groups experienced upper respiratory infections and headaches.

When it comes to the pill form of Semaglutide, the PIONEER-4 trial (which compared oral Semaglutide at 14mg daily to injectable liraglutide) showed these side effects:

  • Nausea: 20%
  • Diarrhea: 15%
  • Vomiting: 9%
  • Constipation: 8%
  • Abdominal pain: 6%
  • Dyspepsia: 6%

Another important study, the SUSTAIN-6 trial, looked at Semaglutide doses of up to 1mg weekly over 104 weeks.

They found it increased heart rate by an average of 2.4 beats per minute from baseline. While this might feel noticeable, research shows it doesn’t pose health risks.

How Long Does It Take for Semaglutide to Work?

Most people start seeing results within the first few months of treatment.

Clinical studies show a consistent pattern of progressive weight loss:

  • By 3 months (12 weeks): Average weight loss of 5.9%
  • By 6 months (28 weeks): Average weight loss of 10.9%
  • By 68 weeks (approximately 16 months): Average weight loss of 14.9%

Weight loss appears to be sustained with continued treatment, with the largest initial losses happening in the first six months.

Over time, the rate of weight loss typically slows but continues to progress.

What Is Tirzepatide?

Tirzepatide (also known by its research code LY3298176) is a groundbreaking medication that works in two ways simultaneouslyโ€”something we call a “dual agonist.”

It works on both the GLP-1 receptor and the GIP receptor, which are important switches that help control blood sugar and weight.

Tirzepatide is a string of 39 amino acids based on a natural hormone in your body called GIP. Scientists added a fatty acid chain (called a C20 fatty di-acid moiety) that helps the medication last longer in your body.

That’s why you only need to take it once a week instead of daily.

In May 2022, Tirzepatide (โ€œMounjaroโ€) was FDA-approved for treating type 2 diabetes. This was a big deal because it was the first medication ever approved that could target both GLP-1 and GIP receptors simultaneously.

In November 2023, it also received FDA approval for chronic weight management (โ€œZepboundโ€). It is effective for weight loss in people with a BMI of 27 or higher.

Now, if youโ€™re interested in how these compounds work:

When you eat, your intestines naturally release two hormones – GLP-1 and GIP. These hormones stimulate your pancreas to produce insulin, which helps control your blood sugar.

Tirzepatide mimics these hormones, but with a twist: while it works like natural GIP, it has five times less affinity for GLP-1 receptors than your body’s natural GLP-1.

This reduced GLP-1 binding helps minimize common side effects like nausea and vomiting while still maintaining powerful weight loss benefits.

Tirzepatide also increases adiponectin in the body. This hormone helps regulate how the body processes fats and sugars.

This might be why Tirzepatide could also help protect the heart.

Tirzepatide Peptide Dosage

Unlike many other peptides, Tirzepatide’s dosing schedule is set in stone through extensive clinical research.

Here’s the standard dosing protocol that doctors follow:

  • Weeks 1-4: Start at 2.5mg weekly
  • Weeks 5-8: Increase to 5mg weekly
  • Weeks 9-12: Move up to 7.5mg weekly
  • Weeks 13-16: Increase to 10mg weekly
  • Weeks 17-20: Bump up to 12.5mg weekly
  • Week 21 onwards: Reach maximum dose of 15mg weekly

All doses are given once weekly as an injection under your skin (subcutaneous).

Important: If you miss a dose, you have a 5-day window to take it. After that, skip it and stick to your regular schedule. Never exceed 15mg per week.

Although this is the official protocol, in my experience, I suggest you might not need such high doses.

Going above 5mg weekly could be unnecessary for most healthy individualsโ€”you’d likely lose your appetite completely.

In fact, I have seen people getting significant results with just 2.5mg per week.

Unless you’re dealing with severe obesity or difficult-to-control diabetes, you probably won’t need those higher doses.

Tirzepatide Cost

Tirzepatide isn’t available without a prescription, and you can’t purchase it online through regular channels.

If you’re looking at pharmacy prices, you may have to pay around $1,000 monthly for four weekly doses.

According to a comprehensive value analysis published in Diabetes, Obesity and Metabolism journal, here’s what the numbers look like:

  • A full 72-week treatment course costs approximately $17,527
  • The annual drug cost runs about $12,658
  • Weekly costs average around $243
  • You’ll spend about $985 (ranging from $908 to $1,075) for every 1% of weight loss achieved

The good news here is that Tirzepatide is more cost-effective than Semaglutide. It costs significantly less per percentage of weight loss achieved.

There are also ways to make it more manageable.

Many insurance providers offer coverage for Tirzepatide, and with a good copay arrangement, you could reduce your out-of-pocket costs.

Tirzepatide Long-Term Side Effects

Thanks to extensive research, including phase 3 clinical trials and real-world monitoring, we clearly understand what side effects you might experience with Tirzepatide.

Most side effects are similar to what you’d see with Semaglutide.

A comprehensive meta-analysis of 6,836 participants across ten trials showed that digestive system issues were the most common complaints.

The higher your dose, the more likely you are to experience side effects:

  • 5mg dose: 39% experienced digestive issues
  • 10mg dose: 46% had side effects
  • 15mg dose: 49% reported problems

The most common side effects were:

  • Nauseaย 
  • Diarrhea
  • Indigestion
  • Headaches
  • Constipation
  • Stomach pain
  • Vomiting
  • Injection site reactions (redness or itching)

About 10% of people on the highest dose (15mg) ended up stopping treatment due to side effects.

The research also found that mild low blood sugar (below 70mg/dL) was most common with the 10mg dose, affecting about 22.6% of users.

Severe low blood sugar only happened when people took Tirzepatide alongside other diabetes medications.

The longest study so far ran for 72 weeks, where:

  • No thyroid cancer cases were reported
  • Serious side effects weren’t any more common than with placebo

In short, Tirzepatide is safe for long-term use when properly prescribed and monitored.

How Long Does It Take for Tirzepatide to Work?

Based on a 2024 large-scale clinical research comparing Tirzepatide and Semaglutide in over 18,386 matched patients, here’s what you can expect in terms of weight loss timeline:

3 Months (12 weeks):

  • You’ll likely see a 5.9% reduction in body weight
    • This means if you start at 200 pounds, you might lose about 12 pounds

6 Months (24 weeks):

  • Weight loss typically increases to about 10.1% of your body weight

12 Months (48 weeks):

  • You could see up to 15.3% reduction in body weight
  • Tirzepatide outperforms Semaglutide by 6.9% at this point

Important Success Rates:

  • About 81.8% of people achieve at least 5% weight loss within a year
  • 62.1% reach 10% or more weight loss
  • An impressive 42.3% achieve 15% or greater weight loss

These results were even better in people without type 2 diabetes compared to those with diabetes, though both groups saw significant improvements.

What’s Better: Semaglutide or Tirzepatide?

The answer isn’t one-size-fits-all.

Let’s look at the evidence-based advantages of each medication.

Is Semaglutide Better than Tirzepatide?

Semaglutide has the advantage of being an established medication with years of proven safety data, making doctors more comfortable prescribing it.

It is also approved for adolescents aged 12 years and up.

More importantly, the SUSTAIN-6 study (mentioned above) found that Semaglutide reduces the risk of heart attacks, strokes, and other major cardiovascular events by 26% in high-risk diabetes patients.

The SELECT trial also found that Semaglutide (2.4mg weekly) reduces the risk of major cardiovascular events by 20% in non-diabetic patients with pre-existing cardiovascular disease.

This unique benefit has not yet been proven with Tirzepatide.

Is Tirzepatide Better than Semaglutide?

Tirzepatide is clearly the superior choice if cost isn’t a major concern for you.

In fact, I’ve previously explained why I consider Tirzepatide to be “Semaglutide 2.0” for fat loss and insulin control in a comprehensive analysis.

According to a 2024 study published in the Journal of Managed Care & Specialty Pharmacy, Tirzepatide shows advantages over Semaglutide:

  • Better Blood Sugar Control: Tirzepatide 15mg reduced HbA1c (blood sugar) by 2.70% compared to Semaglutide 2.0mg’s 2.30% reduction.
  • Greater Weight Loss: Patients lost about 5.15kg (11.3 lbs) more with Tirzepatide 15mg than with Semaglutide 2.0mg.
  • Better Long-Term Outcomes: The study found that Tirzepatide 15mg provided an additional 0.121 quality-adjusted life years compared to Semaglutide!

Also, thanks to its superior results, over 80% of patients would likely find Tirzepatide more cost-effective (even though it costs more outright) than Semaglutide in the long run.

Even when researchers tested these findings under different scenarios and periods (5-year, 10-year, and longer), Tirzepatide consistently outperformed in terms of overall benefits versus costs.

You donโ€™t necessarily need to use these two medicines together because:

  • Combined use increases side effect risks
  • No research supports the safety of concurrent use
  • Insurance typically won’t cover both medications
  • It’s not cost-effective
