The Magic Of Cialis: Why Nitrogen Oxide Stimulation Is the Key To Healthy Aging

Cialis may seem like the “alternative” drug for the ~30 million men in the United States suffering from erectile dysfunction (ED) when the more famous Viagra is not immediately available (Source).

This doesn’t make it inferior… not by any means, and the numbers prove it:

  • In 2019, Cialis occupied 30% of the ED drug market (versus 65% for Viagra)
  • In 2018, nearly 1.9 million prescriptions of Cialis were filled

But I’m about to let you in on a little-known “insider secret” from the fitness industry…

Cialis has a WIDE SPECTRUM of health benefits for any aging male who wants to become fully optimized, especially if they’re using therapeutic testosterone!

I rank it just a shade below Metformin as the most effective anti-aging medication in existence, and it’s a vital part of my daily supplement stack.

Sure, it can enhance erectile strength… but as you’ll see in this article, it can do a hell of a lot more for adding on years of healthy and well-lived life.

What is Cialis?

Cialis (the brand name for Tadalifil) is a prescription medication approved by the FDA to treat 3 specific conditions:

  • Erectile dysfunction
  • Pulmonary arterial hypertension (PAH)
  • Benign prostatic hyperplasia (BPH)

Much like Viagra (sildenafil), it came about in the 1990s when drugs were being designed to inhibit an enzyme PDE5 (more on that later) in the hopes of treating heart disease.

As most people know, the famous story is that one of Viagra’s discovered “side effects” during initial clinical trials was its ability to induce an erection via PDE5 inhibition:

“After these discoveries, sildenafil (Viagra) was approved for the treatment of MED [male erectile dysfunction].

Despite the efficacy of sildenafil, clinically significant adverse effects have been noted, and it has been proposed that some of these side effects may be due to a lack of selectivity for PDE5.

Notably, visual disturbances could stem from inhibition of the retina-specific PDE enzyme, namely, PDE6, involved in vision signal transduction”

Tadalafil ended up being several thousands of times more selective for PDE5 than Sildenafil, which explains why patients report an improved psychological experience and a reduced frequency of side effects such as flushing and vision problems.

But at the end of the day, many people will swear one of them works better than the other for ED.

None of that matters because they fundamentally work through the same biological mode of action.

How Does Cialis Work In The Human Body?

Cialis is best known as a phosphodiesterase type 5 (PDE5) inhibitor, which puts it in the same class of drugs as Viagra and Levitra.

This makes it a vasodilator, which describes a drug that ‘opens’ your blood vessels for increased blood flow).

I’ve provided a simplified explanation of the biochemistry behind what starts and stops erections in The TOT Bible, which I’ll re-post here:

“When a man is sexually stimulated, nitric oxide is released into the penis. Nitric oxide enables the production of cGMP (Cyclic guanosine monophosphate), which controls the dilation (opening) and contraction (closing) of the blood vessels that carry blood to and from the penis.

Another substance, PDE5 (phosphodiesterase type 5 inhibitor), destroys cGMP. When this occurs, the blood vessels return to their normal size and the erection ends.

Cialis stops PDE5 from destroying cGMP, thereby causing the erection to last longer.

Cialis also works by increasing nitric oxide (NO) formation throughout the entire body. This causes vasodilation (opening of blood vessels) and improved blood flow to many tissues, including the brain.”

So in effect, the improved blood flow is due to Cialis inducing the release of NO, which itself is a vasodilator.

However, many people make the common mistake of believing an erection will magically arise and stay stiff by doing nothing else but taking Cialis/Viagra.

This could not be further from the truth if you understand the science behind staying hard:

“Sexual stimulation triggers the release of nitric oxide (NO), stimulating the release of guanylyl cyclase, leading to an increase in intracellular cyclic guanosine monophosphate (cGMP) concentrations, a decrease in intracellular calcium, and ultimately relaxation of the vascular smooth muscle in the corpus cavernosum and penile erection. The PDE5 inhibitors have no effect on the penis in the absence of sexual stimulation.”

No sexual arousal, no release of NO, nothing for Cialis to “enhance”, which means no erection due to a lack of penile blood flow and adequate vasodilation.

This is exactly why people with poor blood flow and/or medical conditions involving suppressed or non-existent NO activity (such as diabetes) find Cialis to be a lot less effective!

Top 6 Health Benefits Of Cialis

Cialis has been one of the earliest Golden Age medications I have recommended to my readers.

I’ve had the privilege of using it for over 15 years now, benefitting from it in numerous ways, and continue to do so without any negative health outcomes.

Cialis truly has positive effects beyond ED and I want you to know all about them.

Because once you do, you’ll be racing to your doctor’s office for a lifetime prescription.

Cialis is Beneficial For Heart Health

Cialis doesn’t just protect your sex life; it’s also cardio protective, which means it provides your body’s entire vascular network with protection.

While the majority of the research in this area has been done on Viagra, it’s not unreasonable to say the results could extend to Cialis use, given that the two drugs work through the same mechanism of action.

Especially given that “tadalafil is not associated with increased risk for clinically significant cardiovascular events” (Source)

As it turns out, there are multiple avenues through which Cialis can keep your heart pumping and free of build-up:

“It improves peak oxygen consumption, reduces minute ventilation/carbon dioxide output slope, and acts as a selective pulmonary vasodilator during rest and exercise in patients with heart failure and pulmonary hypertension.

It improves flow-mediated maximal dilatation, ergoreflex on ventilation, and breathlessness both after 3 and 6months of treatment.

Sildenafil, by improving endothelial activity and muscle perfusion, improves signaling and ventilatory efficiencies, potentially indicating a novel approach in the therapeutic management of heart failure.”

This has also been evidenced in pre-clinical studies looking at a wide variety of heart conditions:

“A number of preclinical studies have shown that PDE5 inhibitors have powerful protective effect against several clinical scenarios including myocardial ischemia/reperfusion injury, doxorubicin and post-MI, heart failure, cardiac hypertrophy, heart transplantation, Duchenne muscular dystrophy and preconditioning of stem cells.

Based on these studies, it appears that sildenafil and other PDE5 inhibitors hold promise for further development as novel drug therapies in cardio protection.”

Add in the reduction of heart inflammation and you have a drug with great clinical potential to prevent heart disease and reduce the probability of heart attacks.

It’s no wonder that Eli Lilly, the company that originally manufactured Cialis, is now looking into Cialis for treating heart failure!

Less plaque buildup in the arteries, better blood flow, increased nitric oxide stimulation, improved endothelial function… all things essential for a healthy beating heart.

Cialis Lowers Blood Pressure

As you can imagine from what you just learned about Cialis and heart health, someone suffering from hypertension may benefit from daily Cialis use.

This is precisely what Japanese health regulators discovered when post-marketing surveillance data was collected on Cialis in 2017:

“Among 1556 patients analyzed for effectiveness, the percentages of patients with improvement of WHO functional class at 3 months, 1 year, and 2 years after the initiation of tadalafil, and last observation were 17.1%, 24.8%, 28.9%, and 22.5%, respectively.

At all observation points (except pulmonary regurgitation pressure gradient at end diastole at 3 months), the mean 6-minute walk distance, cardiac catheterization, and echocardiogram measurements showed statistically significant improvement.”

This surveillance demonstrated that tadalafil has favorable safety and effectiveness profiles for long-term use in patients with PAH [pulmonary arterial hypertension] in Japan.”

So for individuals with normal blood pressure who are looking to improve their current numbers, Cialis may be useful. 

Even in short-term situations, a single 20 mg dose of Cialis has been shown to lower ambulatory blood pressure in patients suffering from hypertension.

As a matter of fact… a separate FDA-approved drug called Adcirca contains tadalafil as the primary active ingredient and is indicated for this very purpose:

“Tadalafil is an oral medication called a phosphodiesterase-5 (PDE5) inhibitor approved for the treatment of pulmonary arterial hypertension (PAH) in World Health Organization (WHO) Group 1 patients.

The goal of this therapy is to improve exercise ability and delay clinical worsening. Research studies showing the effectiveness of the medication included mostly patients with symptoms that were rated as WHO Functional Class II-III.

Tadalafil is marketed as Adcirca® for PAH and was approved by the United States Food and Drug Administration (FDA) in 2009

The recommended dose of tadalafil for the treatment of PAH is 40 mg (two 20 mg tablets) once daily with or without food.”

I guess that’s why some people refer to Tadalafil as “the Aspirin of the future”:

“This drug actually improves the health of our body’s largest organ, the Endothelium, our blood vessels’ inner lining. By reducing blood pressure and decreasing the inflammatory markers, which indicate hardening of the arteries, it seems that Tadalafil easily trumps Aspirin as a protector of the heart and blood vessels.”

Who knows… maybe one day we’ll all be carrying a few tablets of Cialis with us when we travel! (And maybe I carry ‘Liquid Cialis’ with me everywhere I go.)

Cialis Could Enhance Physical Performance

In my early days of using Cialis as a single man, I remember one night where I went to the gym shortly after taking the drug.

I felt a phenomenal pump effect in my muscles — far too big for me to write it off as a placebo.

As I went home to research what happened, it turns out there was something to my accidental discovery.

In my mind, Cialis’ ability to act as a vasodilator and increase blood flow was also applicable to muscle cells – that blood flow goes to muscles and organs as well, which explained why I noticed a visible boost in performance.

The evidence for this health benefit is sparse but indeed promising.

In one study where non obese men suffering from ED used 5 mg of tadalafil a day for two months, a small amount of lean muscle tissue was gained:

“Daily tadalafil improved lean mass content in non-obese men probably via enhanced insulin secretion, estradiol reduction, and improvement of endothelial function in vivo. The in vitro increased myogenin and androgen receptor protein expression in skeletal muscle cells suggests a translational action of phosphodiesterase type-5 on this receptor.”

Other possible mechanisms of action worth reviewing include…

  • Increased oxygen supply to muscles, according to two human studies (here and here) examining athletes at unusually high altitudes where oxygen is scarce
  • Possible reduction of muscle fatigue and increased protein synthesis, according to a very small human study involving 20 mg of Viagra once a day over 8 days
  • Improved ratio of testosterone to estrogen with long-term daily use of Cialis, as discovered in two small pilot studies involving men (here and here)

A great benefit for both younger and older men who want to push themselves harder and longer in the gym!

Cialis May Improve Cognitive Function

Believe it or not, some emerging evidence suggests Cialis may also be neuroprotective (i.e beneficial for brain health).

In 2019, 25 male older-age patients suffering from mild cognitive impairment and 90 days’ worth of ED were given 5 mg of Cialis once a day for 8 weeks straight.

Not only did the patients measurably improve their scores for erectile function and cognitive functioning, but there was increased regional cerebral blood flow (more blood supply to the brain).

Surprisingly, a separate yet smaller clinical study on humans done in the same year also made a similar discovery when Cialis was found to lower neuro inflammation:

“Inflammatory, psychometric and electrophysiological parameters were normalized after tadalafil treatment.

In conclusion, there is a pro-inflammatory environment in blood in patients with BPH/LUTS-ED which would induce cognitive impairment and alter MMN [mismatch negativity].

Phosphodiesterase-5 inhibition with tadalafil exerts anti-inflammatory effects and ameliorates cognitive function and MMN parameters.”  

But even before these human studies, there was a growing interest in re-purposing ED drugs like Cialis for enhanced cognition:

“In preclinical studies, PDE5 inhibitors protect against cognitive impairment and neurodegeneration.

Some proposed mechanisms include reduced tau tangles (a component of Alzheimer’s disease), increased protective molecules such as brain-derived neurotrophic factor (BDNF), reduced inflammation, and reduced cell-death signaling, but none of these effects have been demonstrated in humans yet.”

Cialis Will Give You Stronger, Longer-Lasting Erections

When it comes to getting hard and staying hard, there are four types of impotence men commonly experience:

  • You can’t get an erection, period.
  • You get semi-hard, but not fully hard (i.e. insufficient for penetration)
  • You get fully hard but you can’t stay hard for long enough
  • You spontaneously get an erection on occasion, but not always when you need it most

Fortunately for us men, Cialis is one of the most proven medications in existence for improving blood flow to sexual organs while heightening their sensitivity to stimulation.

And given how performance anxiety can get in the way, or erectile strength generally gets weaker with older age, it’s good to have a useful tool for enhanced sexual functioning.

I probably don’t need to go over the clinical evidence with you given the FDA seal of approval for treating ED with Cialis, but let’s summarize it briefly…

A 2006 review is the best compilation of evidence we have to date on the safety and efficacy of Cialis to treat ED.

There are multiple compilations of trials and human research studies cited, but this one finding says it all:

“Integrated analysis of data from phase III trials demonstrated that tadalafil, at doses from 5 mg to 20 mg versus placebo, significantly improved erectile function (EF) by all measures.

In particular, 50%–65% of patients, regardless of ED severity at baseline, returned to normal erectile function with almost 60%–90% of success rate at intercourse attempts during active treatment.”

Yet the real interesting part is what happens when you look at men who take Cialis DAILY instead of merely when they need it.

A compilation of 8 studies published before 2010 reached the following conclusion about once-a-day dosing of Cialis:

“It is clear from numerous high-quality studies that tadalafil as a daily dose is highly efficacious in the management of ED from a variety of causes. It is also clear that daily dose of tadalafil will have a beneficial effect on the sexual relationship between partners.”

“Side effects from daily dose tadalafil in clinical trials have generally been mild, with 54%–80% of reported events mild in nature and with no more than a third of events moderate in severity. Serious side effects have not been reported in some studies; in studies where serious events occurred, the effect was considered not to be related to the study drug itself”

Not only does it provide the desired results, but one 2019 meta-analysis argues for the superiority of daily Cialis use:

“4 articles, including 1,035 participants were studied. The analysis found that tadalafil daily had a greater improvement than tadalafil on-demand in terms of International Index of Erectile Function-Erectile Function in treating ED after at least 24 weeks treatment cycle.

For safety, tadalafil on-demand had a higher incidence of any treatment-emergent AE compared with tadalafil daily, but for other aspects, including discontinuation due to AEs, myalgia, back pain, headache, dyspepsia, and nasopharyngitis, there were no significant difference between the 2 treatments.”

But enough with the studies, because I can share my personal experience with using Cialis for sexual enhancement: 

I used Cialis on-and-off in daily micro dose fashion since 2008 and I can say it is singlehandedly the greatest recreational drug for a single bachelor man in his mid-30s who has a bunch of women around. 

I had many single guys text me late at night and refer to it as “the best drug in the history of mankind” — I thought the same during the 1.5 years I was divorced and fully single.

My good friend Alexander Cortes agrees with me, having the following to say:

“If you are on TOT and you want to get an erection of steel 3-6 times a night, go out and start using Cialis. It will do wonders for your self-esteem, overall masculine confidence and energy”

An old Youtube comment will drive the point home even further:

“Cialis is awesome! I take 20 mg, have the best sex ever and great pumps at the gym for 3 Days. Can’t beat it, worth every penny and I’m on TOT. Double whammy!!” 

No more has to be said!

But I do want to address one minor point often brought up by people who are hesitant to start therapeutic testosterone: They are worried their penis will shrink.

As I said in The TOT Bible, it doesn’t happen and this belief is based on misinformation:

“…the penis can increase in size and/ or girth because testosterone increases nitric oxide production, which stimulates nitrogen retention and enhances blood flow. All of this leads to harder and thicker erections.

In fact, increasing nitric oxide is one of the things that Cialis does to give men stronger erections.”

Not to mention testosterone is ESSENTIAL for a high-charged sex drive.

Cialis Can Treat Benign Prostate Hyperplasia (BPH)

This is another FDA-approved use of Cialis, which received the green light in 2011.

For those of you who don’t know what BPH is, here’s a quick overview:

“Benign prostatic hyperplasia (BPH) — also called prostate gland enlargement — is a common condition as men get older. An enlarged prostate gland can cause uncomfortable urinary symptoms, such as blocking the flow of urine out of the bladder. It can also cause bladder, urinary tract or kidney problems.”

“…symptoms tend to gradually worsen over time. Common signs and symptoms of BPH include: Frequent or urgent need to urinate, Increased frequency of urination at night, Difficulty starting urination, Weak urine stream, Dribbling at the end of urination, Inability to completely empty the bladder”

Once again, I’ll briefly go over the evidence as the clinical data support Cialis for this purpose is abundant.

Three years after Cialis was approved for BPH treatment, a review of multiple studies found the evidence remained strong:

“The efficacy of tadalafil 5 mg once daily for the treatment of LUTS [lower urinary tract symptoms] has been reported by several well-designed studies. Tadalafil improves significantly the total International Prostate Symptom Score (IPSS), the voiding and storage subscores, the IPSS Quality of Life (QoL) and the BPH Impact Index (BII)…. Its efficacy is irrelevant to the erectile function status of the patients.

…Its safety profile is well established and no new or unexpected adverse events other than those reported in ED studies have been recorded.. However, it is not just an alternative [to already-established drugs], since sexual adverse events associated with these drugs are avoided and tadalafil is the only drug that can treat both ED and LUTS at the same time.”

This effect is believed to happen because Cialis relaxes the smooth muscles of the prostate, which leads to the reduction of BPH symptoms.

This is good news for therapeutic testosterone patients who fear the possibility of prostate enlargement, even though I’ve thoroughly proven this idea to be unequivocally false in The TOT Bible.

Long story short: Daily Cialis use can help older men achieve a better urine stream. No longer do they need to wake up 2-3 times in the middle of the night to pee.

The BEST Dose Of Cialis For Better Sex And A Longer Life

Based on my years of experience in using Cialis for myself and recommending it to my private clients, I know the optimal dosing protocol.

After years of using it I shared it publicly in a 2016 podcast I did with Alexander Cortes and Jim Brown:

My daily dosing recommendation for Cialis is 2.5-7.5 mg taken once a day, at the same time every day, ideally in the morning or right before working out to increase blood flow and improve training performance. 

Used correctly, a daily micro dose of Cialis might be one of the most important life-enhancing medications an aging male can take to confer all the positive health benefits listed above — brain and heart protection, better blow flow to muscles, strengthened erections, etc.

(You’ll also avoid the potential side effects I write about in the next section of this article)

If you want a “sexual boost” protocol during an intimate two-day weekend with your partner: 15-30 mg daily during the “event” and then return to your daily maintenance dose the following week. 

Although most people will take Cialis in tablet form, I personally prefer the liquid formulation. 

One writer summarizes the advantages of liquid form over tablet form, and I’ll list them here as they match my own personal experience with using Cialis:

  • More portable, and need to take liquid Cialis with water
  • Easier to swallow than a tablet
  • Faster-acting due to absorption inside the stomach

Additionally, I have separate recommendations for how men should use Cialis depending on how old they are:

For men in your 20s, you can use Cialis to improve your numbers in the gym… you’ll also experience the benefit of longer-lasting erections, although you shouldn’t NEED to rely on the medication in that age bracket.

For older men in their late 30s and 40s, use Cialis to keep themselves healthier for longer while having a higher quality of life.

Finally, keep in mind that numerous factors can affect how long Cialis takes to “kick in” and how effective it will be.

I recommend reading these two guides from Healthline and Harvard Health Publishing for more intel on maximizing Cialis’ effects, but pay close attention to the following:

  • Alcohol consumption
  • Cigarette use
  • Physical activity, or lack therof
  • Your current stack of medications/supplements
  • Pre-existing co-morbidities such as obesity, diabetes, etc.
  • Age
  • Diet, especially around the time Cialis is taking

Side Effects & Safety Profile of Cialis

Since I am advocating for a “lifestyle” use of Cialis, the low daily dosages I suggest should cause very few side effects (if at all).

But in the interest of full transparency, there are certain side effects you should watch out for:

  • Runny nose
  • Muscle aches
  • Indigestion
  • Flushing
  • Dizziness
  • Headache
  • Nausea

However, since Cialis dilates your blood vessels and consequently changes your body’s blood pressure, the above side effects are in line with what is expected to happen.

And given its long-standing FDA approval status of nearly 2 decades, we have an abundance of evidence showing long-term daily use of Cialis is safe.

Such as this 2004 paper that studied 1173 men with ED who were taking other medications for prior conditions (hypertension, diabetes) while using Cialis daily for 18-24 months.

The results were mind-blowing:

“Tadalafil was safe and well tolerated over 1676 patient years”

“The rate of discontinuations due to adverse events for this 18–24-month study was 6.3% and the rate for any individual event was <1%”

“No consistent pattern of serious adverse events assessed as causally associated with tadalafil administration was observed”

Despite all this good news, there are certain medications you should explicitly avoid taking if you’re going to use Cialis:

“…rifamycins, antibiotics, antifungals, antidepressants, barbiturates, drugs to treat high blood pressure or a prostate disorder, heart or blood pressure medications, HIV or AIDS medications or seizure medications.”

Furthermore, the major warning I would give out is to avoid Cialis use altogether if you’re already using nitrates for chest pain and/or heart problems:

“This includes nitroglycerin, isosorbide dinitrate, and isosorbide mononitrate. Nitrates are also found in some recreational drugs such as amyl nitrate, butyl nitrate or nitrite (“poppers”). 

Taking tadalafil with a nitrate medicine can cause a sudden and serious decrease in blood pressure.

Some tadalafil can remain in your bloodstream for 2 or more days after each dose you take (longer if you have liver or kidney disease).

Avoid nitrate use during this time.”

Start with the LOWEST daily dose possible (2.5 mg) and slowly work your way up to 7.5 mg.

And like any new drug you introduce into your daily regimen, consult with your physician first before making any final decision on the use of Cialis.

Cialis Vs. Viagra Vs. Levitra: What’s The Difference?

You might be wondering whether Cialis’ numerous lifestyle benefits can be obtained by using other ED drugs such as Viagra (sildenafil) and Levitra (vardenafil).

My personal experience with all three drugs would have to be NO — Cialis is the only one that can reliably be dosed daily in a micro dose fashion and have additional uses outside of improving one’s ability to get and maintain a firm erection.

You’ll see why as I compare the good, the bad, and the ugly three drugs below…

Similarities Between Viagra, Cialis, and Levitra

Function: All three drugs are PDE5 inhibitors that treat erectile dysfunction via the same mechanism of action.

Official Status: All three drugs have been FDA-approved to treat erectile dysfunction — Viagra in 1998, Levitra in 2003, Cialis later that same year 2003 (Source).

Effectiveness: When compared head-to-head in randomized controlled trials, “improvement of ED and adverse events did not differ among treatments” (Source)

Side Effect Profile: The following side effects are consistent across all 3 medications — headache, runny nose, back pain, upset stomach, flushing (Source).

Differences Between Viagra, Cialis, and Levitra

Onset and Duration of Effect (Source):

  • Viagra — taken 30 minutes before sex, lasts between 4-8 hours in the body
  • Cialis — taken 30 minutes before sex, lasts 18-36 hours in the body
  • Levitra — taken 1 hour before sex, kicks in slightly faster than Viagra lasts between 4-8 hours in the body

Taking With/Without Food (Source):

  • Viagra — On an empty stomach, as a fat-containing meal will lower its effectiveness
  • Cialis — Can be taken either with/without food
  • Levitra — Can be taken either with/without food

Side Effect Profile (Source):

  • Viagra — uniquely has the rare side effects of vision changes (“blue vision”) and skin rash
  • Cialis — uniquely has the rare side effect of limb pain
  • Levitra — has the same side effects as Viagra but without the rare ones just mentioned.

Relative Cost (Source):

  • Viagra — Lowest
  • Cialis — Medium
  • Levitra — Highest

Although some experts have brought forth the idea of using generic Viagra (sildenafil) on a daily basis, I think the clear winner here is Cialis.

Its effects last longer, it can be taken with a meal, it has a solid and well-research safety profile, and its price tag isn’t significantly different from that of Viagra.

But as you’ll see in the next section, there’s an even better way to get it.

Where To Buy Prescription-Strength Cialis Online

Unfortunately, Cialis is a prescription drug and thus you can’t get it unless a physician deems it necessary for the treatment of erectile dysfunction or BPH.

And getting it through “official” means can be awfully expensive:

 “…ever since Cialis went generic in October 2018, multiple drug makers have released their own generic versions…This is promising news for tadalafil prices, since more independent options means more market competition and lower prices”

The average cash price for a common prescription of generic tadalafil (thirty 5 mg tablets) is now about $300, or $10 per pill. That’s roughly a third less than what brand-name Cialis would cost for the same prescription (about $455).

With a GoodRx coupon, you might be able to pay as little as $9 for thirty 5 mg tablets of tadalafil (30 cents per pill)”

While I am not endorsing GoodRx directly, it is in your best interest to get generic tadalafil and explore all your options for lowering the cost (i.e. insurance, bulk prescriptions, choosing low-cost pharmacies, etc.).

You probably already know this, but it’s worth a friendly reminder that generic and brand name drugs work in the exact same way:
