Most men over 40 are flying completely blind into their later years.

And what’s fooling them the most is the standard blood panel their physician orders them to get during every annual checkup.

When the values come back “normal,” the patient thinks nothing of it and truly believes everything is just fine.

But as soon as they exit the doctor’s office, they make a swift return to reality.

They feel exhausted, their body composition is shifting despite their best efforts, their drive and libido are quietly eroding, and they can’t stop feeling foggy at 2pm every day. 

Why is that?

Simply put, the standard blood panel your conventional physician orders has nothing to do with optimize human performance. 

And it’s most likely missing the key biomarkers for men over 40 who want more out of life.

As someone who’s been testing my own blood obsessively for over 25 years, working alongside some of the most forward-thinking clinicians and researchers on the planet, I can tell you this straight to your face:

The difference between men who thrive past 40 and men who slowly decline is almost always traceable to specific biomarkers that conventional medicine ignores.

Why “Normal” Lab Results Are Keeping Men Sick

The reference ranges printed on your lab report represent a statistical average of a metabolically compromised population that is both sick and sedentary.

Being “normal” in that context is not a compliment.

When your total testosterone comes back at 320 ng/dL and the reference range says 264-916 ng/dL, your doctor sees no cause for concern. 

You, my dear reader, ARE NOT FINE!

I want you to internalize this distinction because everything that follows depends on it:

We are chasing OPTIMAL and deliberately falling into the range of “average”.

The 15 Biomarkers For Men Over 40 To Monitor Annually

1. Total Testosterone

Total testosterone measures the entire circulating pool of testosterone in your blood, including both bound and unbound fractions.

Most labs flag anything above 264 ng/dL as normal.

My position, shared by every elite clinician I respect, is that optimal total testosterone for a man over 40 *generally* sits between 700 and 900 ng/dL.

Above roughly 916 ng/dL is when you enter supraphysiologic territory by current assay reference ranges, which is a different conversation with different tradeoffs.

Anything below 500 ng/dL, however, warrants serious investigation.

2. Free Testosterone

Free testosterone represents the fraction not bound to sex hormone-binding globulin (SHBG) or albumin, which makes it the biologically active portion available to your cells.

The free hormone hypothesis is the governing principle of androgen physiology, and it has been validated in vivo using transgenic models where raising SHBG blunted androgen bioactivity despite higher total levels.

You can have a perfectly “normal” total testosterone and genuinely low free testosterone if SHBG is elevated.

Optimal free testosterone sits between 20 and 25 pg/mL for most men.

3. SHBG (Sex Hormone-Binding Globulin)

SHBG is the protein that binds testosterone and renders it biologically inactive.

As men age, SHBG naturally rises, which suppresses free testosterone even when total testosterone appears unaffected.

High SHBG is often driven by factors such as elevated estrogen, thyroid dysfunction, or chronic caloric restriction.

Target SHBG between 20 and 40 nmol/L.

The opposite problem matters equally, and I covered what low SHBG actually signals in its own breakdown.

4. Estradiol (Sensitive Assay)

Estradiol (E2) is the primary estrogen in men, produced through the conversion of testosterone via the enzyme aromatase.

This biomarker must be measured using the sensitive LC/MS assay, and NOT the standard immunoassay many labs run by default as it is notoriously inaccurate for male physiology.

Optimal estradiol levels for men sit between 20 to 50 pg/mL on the sensitive assay.

E2 levels too low cause joint pain, cognitive decline, loss of libido, and bone loss (you DO NOT ever want to artificially suppress estrogen production)

Whereas E2 levels too high drive fat gain, water retention, and emotional dysregulation.

Now here is what most people get wrong, and I have said this in every book I have written:

There is no cookie-cutter E2 number that applies to every man, and how you feel outranks where you land in a range.

5. LH and FSH

Luteinizing hormone (LH) and follicle-stimulating hormone (FSH) are the pituitary signals that drive testosterone and sperm production.

These markers tell you whether a testosterone deficiency originates in the testes (primary hypogonadism) or in the brain (secondary hypogonadism).

This distinction must be made as it will dictate which intervention will actually end up working for you.

For instance: Low LH alongside low testosterone points to a signaling failure at the hypothalamic-pituitary axis, rather than testicular failure.

6. IGF-1 (Insulin-Like Growth Factor-1)

IGF-1 is the primary downstream mediator of growth hormone (GH) activity.

Because GH is pulsatile and difficult to measure directly, IGF-1 gives you a stable look into GH axis function.

Declining IGF-1 is associated with accelerated aging, reduced muscle mass, increased visceral fat, and impaired tissue repair.

Optimal IGF-1 for men over 40 falls between 200 and 300 ng/mL.

Understand the ceiling here, because more is not better when it comes to IGF-1, and it should be optimized without exceeding the top of the normal range.

If your IGF-1 is chronically under 150, that is a conversation worth having with an optimization-minded physician about growth hormone secretagogues.

7. Thyroid Panel (TSH, Free T3, Free T4, Reverse T3)

A standard thyroid test orders thyroid-stimulating hormone (TSH only) which is completely inadequate and tells you almost nothing about what is actually happening at the cellular level.

You need Free T3, Free T4, and Reverse T3 at a bare minimum.

Target TSH under 2.5 uIU/mL and Free T3 above 3.7 pg/mL.

Free T3 is the active hormone your cells actually use.

Reverse T3 is an inactive isomer that can competitively inhibit T3 at receptor sites, effectively braking your metabolism while your TSH still reads normal.

A high rT3 to T3 ratio explains a huge share of the fatigue, cold intolerance, and metabolic slowdown that gets dismissed in nine-minute appointments.

8. Fasting Insulin

This single marker is one of the most predictive biomarkers for long-term metabolic health, yet almost no conventional physician orders it.

Fasting insulin reveals loss of insulin sensitivity long before HbA1c or fasting glucose ever moves.

The Whitehall II cohort tracked people all the way to diagnosis and showed insulin sensitivity declines years ahead of any glucose abnormality.

Optimal fasting insulin is below 5 uIU/mL, and anything above 10 demands immediate metabolic intervention.

9. HbA1c (Glycated Hemoglobin)

HbA1c reflects your average blood glucose over roughly 90 days.

Unlike a single fasting glucose measurement, it captures the chronic metabolic environment your cells are actually living in.

Optimal HbA1c for longevity sits between 4.8 and 5.3 percent.

The conventional cutoff of 5.7 percent is far too permissive if you are optimizing for the long game (and let’s be honest, you’re already prediabetic by that point in time).

10. Fasting Glucose

A simple yet foundational marker that is part of virtually any panel ordered by a doctor, whether they are part of sick care or optimization care.

Optimal fasting glucose is 70 to 85 mg/dL.

Creeping values between 90 and 99 mg/dL get labeled normal, but signal early metabolic dysfunction when paired with elevated insulin.

Never evaluate fasting glucose in isolation, and always pair it with fasting insulin every single time to get the full picture of what’s happening in your body. 

11. High-Sensitivity C-Reactive Protein (hs-CRP)

High-sensitivity C-reactive protein is a primary marker of systemic inflammation, the silent driver behind cardiovascular disease, cognitive decline, hormonal disruption, and accelerated aging.

Ridker and colleagues moved hs-CRP from concept to clinical benefit by showing it predicts events at every level of LDL cholesterol, along with every level of conventional risk scoring.

Optimal hs-CRP is below 0.5 mg/L, and anything above 1.0 mg/L demands a root-cause investigation.

Chronic elevation points to poor sleep, gut dysfunction, dietary inflammatory load… and possibly a smoldering infection nobody has looked for up until now.

12. Homocysteine

Homocysteine is an amino acid produced during methionine metabolism.

Elevated homocysteine is an independent risk factor for cardiovascular disease and is strongly associated with both endothelial damage and cognitive decline.

Target homocysteine below 7 umol/L.

Conventional medicine accepts up to 15 umol/L, but that ceiling is way too high.

Elevated homocysteine typically responds well to targeted B-vitamin therapy; specifically methylfolate, methylcobalamin, and B6.

13. Ferritin

Most people know ferritin as an iron storage marker.

What they do not know is that ferritin is also an acute phase reactant, meaning hyperferritinemia tracks inflammation independently of how much iron you are actually storing.

Optimal ferritin for men is 55 to 270 ng/mL.

Do not let anyone phlebotomize you into iron deficiency in the pursuit of chasing a lab number, because ferritin below 100 ng/mL starts costing you thyroid function and red cell production.

On the other end, elevated ferritin in a man with no iron deficiency correlates with insulin resistance and metabolic syndrome.

Therfore, treat it as an inflammation signal rather than proof of healthy iron stores.

14. 25-OH Vitamin D

25-hydroxyvitamin D is the storage form of vitamin D measured in the blood.

Vitamin D is more of a steroid hormone than it is a vitamin (technically speaking), and the vitamin D receptor is expressed across 30+ tissues and organs where it regulates gene transcription directly.

Most labs call anything above 30 ng/mL sufficient.

I personally target 70 ng/mL or above for true optimization, typically with 5,000 to 10,000 IU daily.

NEVER supplement vitamin D without also optimizing vitamin K2 and magnesium, as they are the cofactors that determine whether D3 helps you or calcifies your arteries.

15. Advanced Lipid Panel (ApoB, LDL-P, Lp(a))

Standard total cholesterol and LDL-C measurements are blunt outdated tools for cardiovascular risk assessment.

Apolipoprotein B and LDL particle number measure the actual number of atherogenic particles circulating in your blood, and discordance analyses consistently show ApoB outperforms LDL-C when the two disagree.

Lipoprotein(a) is a largely genetically determined causal cardiovascular risk factor that roughly 20% of people carry at elevated levels.

To nobody’s surprise, the vast majority of men have never had this marker measured once.

You cannot manage what you do not measure.

Get Lp(a) tested one time and know where you stand for life.

Safety, Risk, and Important Considerations

These biomarkers are diagnostic tools and should not be regarded as standalone prescriptions for action.

Interpreting labs in isolation without your symptoms, history, lifestyle, and the full clinical picture is how men make expensive mistakes.

And to this point: DO NOT self-prescribe hormone therapy, thyroid medication, or any pharmaceutical intervention based on a single lab draw.

Find an optimization-minded physician who can accurately read these markers through a performance and longevity lens

Lab testing should be done at least twice per year if you are actively optimizing, and quarterly if you are evaluating the effects of a new therapeutic protocol.
