But you lost the body composition war because you had to sacrifice a decade’s worth of hard-earned muscle tissue.
What you engaged in was a trade that works against you: A metabolic catastrophe masquerading as success.
And it’s a trade millions of people are taking when they use GLP-1 receptor agonists like Semaglutide and Tirzepatide.
Once reality hits them straight between the eyes, they search up “GLP-1 muscle loss” on Google to figure out where they went wrong.
To make matters worse, most prescribing physicians are either unaware of this phenomenon or greatly downplay its significance within the context of overall metabolic health.
I’ve watched too many men and women drop 30-50 pounds and emerge from their weight loss journeys physically weaker with far less muscle mass.
Just to add the cherry on top, these people now have a hormonal profile worse than the one they started with.
There is a smarter way to use these tools, and this article is going to reveal it to you.

The Dirty Secret Inside Every GLP-1 Prescription
Most people reading this already know GLP-1 receptor agonists like Semaglutide, Liraglutide, and Tirzepatide work primarily by mimicking glucagon-like peptide-1 (GLP-1).
And if you don’t already know, GLP-1 is a gut-derived hormone that slows gastric emptying, reduces appetite, modulates insulin secretion, and lowers glucagon secretion.
What your prescribing physician is probably not telling you is what happens downstream once caloric intake is lowered due to the appetite suppression.
When you put your body in a sustained caloric deficit for an extended period of time, your body does not selectively mobilize stored fat.
It catabolizes everything available to it as a means of preserving only what is absolutely essential in a state of lower energy availability… including skeletal muscle.
Body composition data shows a large fraction of total weight lost during GLP-1 therapy is lean mass rather than fat, with analyses of the major trials placing that number in the range of 25 to 40%.
BUT: As anybody who has read my magnum opus on GLP-1 peptides already knows, lean mass includes skeletal mass alongside other components (water, bone, organs, etc.).
And a poorly setup lifestyle intervention, especially where protein intake and resistance training are not dialed in, will generate the same losses without the use of a GLP-1 peptide.
That being said: Your muscle mass is valuable metabolic currency that drives insulin sensitivity, glucose disposal, resting energy expenditure, and your long-term healthspan.
Losing it while using a GLP-1 peptide is a reflection of the protocol failing, rather than a direct side effect of the GLP-1 peptide itself.
Why the Mainstream “Eat More Protein” Response Is Incomplete
The standard advice from the medical establishment in response to the problem of muscle mass loss is to eat more protein and do some resistance training.
While it’s high time they started saying this (considering how late to the game they are), a blanket recommendation like this is somewhat incomplete.
Think about it for a second: When you’re using a GLP-1 peptide, appetite suppression makes hitting the required (and oftentimes higher) protein targets extremely difficult for most users.
A reduced caloric intake also suppresses insulin-like growth factor 1 (IGF-1) and blunts mTORC1 signaling, the latter of which is the primary anabolic pathway driving muscle protein synthesis.
Myostatin, the negative regulator of muscle growth, also less inhibitory resistance in a catabolic state.
Cortisol can rise with rapid weight loss, further accelerating muscle breakdown through glucocorticoid receptor activation in skeletal muscle.
What you have to do is attack these leftover “leaks” at the receptor and pathway level with the right compound… IN ADDITION to maintaining the high protein intake and consistent resistance training.
What FLGR-242 Is and Why It Belongs in This Protocol
FLGR-242 is a fragmented and modified version of Follistatin-344 fused to a proprietary albumin-binding construct, which I break down in full in my newest book, Living Leaner Longer Stronger.
FLGR-242 works by neutralizing myostatin directly, releasing the brake on muscle protein synthesis and satellite cell activation at the exact moment a caloric deficit is trying to slam that brake down harder.
Its FS I-I isoform base retains full myostatin-neutralizing power while dramatically reducing activin binding, meaning you get the muscle-preserving signal without the off-target hormonal disruption that ended older myostatin inhibitors.
The albumin binder extends its half-life to roughly 19 days per injection compared to the 1 to 2 hours of raw Follistatin-344, and unlike every earlier attempt at creating a myostatin inhibitor, it delivers a corresponding increase in both muscle size AND strength.
So it should be fairly obvious why I recommend GLP-1 peptide users stack on FLGR-242 right away.
Large human trials specific to FLGR-242 do not exist yet, but I weight documented real-world results and decades of myostatin biology far above waiting years for a regulator’s blessing.
The evidence that matters most (IMO) is case studies from people like IFBB Pro Alex Padron, who added 15 pounds of lean mass while dropping from 12% to 4% body fat across 10 weeks with FLGR-242 as the only variable he changed in his protocol.
It’s the coveted fat-loss-with-muscle-gain outcome every GLP-1 peptide user is chasing.
The Lean Mass Preservation Protocol: Building the Full Stack
Here is how I would structure a complete protocol for anyone on a GLP-1 receptor agonist who is serious about protecting muscle tissue.
While none of this constitutes medical advice, what follows below is what I have guided informed individuals to consider with the support of an optimization-minded physician.
Foundation Layer: Non-Negotiables
- Optimized testosterone (for men)
Low testosterone levels combined with reckless GLP-1 peptide use is a recipe for muscle loss.
If you are not on a well-managed testosterone optimization protocol, you are metaphorically fighting the body composition battle with one hand tied behind your back.
- Protein targets, hit them regardless of appetite
Aim for a minimum of 1 gram per pound of lean body mass daily, using leucine-rich sources or essential amino acid formulas when whole food intake is crushed by the drug.
And as you get leaner, get ready to increase that number to 1.25-1.5.
- Progressive resistance training
This is the primary mechanical stimulus for mTORC1 activation, and controlled trials confirm resistance training is the best non-pharmacological intervention available for preserving muscle mass in dieting adults.
Three to four sessions per week at minimum, built on the right exercises and progressive overload.
Peptide Layer: Strategic Co-Administration
FLGR-242, when used alongside a GLP-1 receptor agonist, helps preserve the anabolic signaling environment that caloric restriction actively suppresses.
But there are other peptides with relevance to muscle tissue in the context of GLP-1 peptide use that are worth knowing about:
- BPC-157 provides systemic repair and anti-inflammatory support, while potentially reducing muscle tissue damage acquired during caloric stress.
- CJC-1295 and Ipamorelin are growth hormone secretagogues that can partially offset the IGF-1 suppression typically associated with caloric restriction
- MK-677 is an oral ghrelin receptor agonist that drives GH and IGF-1, though it can stimulate appetite and partially blunt the GLP-1’s primary mechanism
Use the GLP-1 to control appetite and drive fat loss, then use the peptide layer to defend the anabolic pathways.

Safety, Risks, and Contraindications
I will not pretend these tools are totally free from negative consequences.
GLP-1 agonists carry well-documented risks that include nausea, vomiting, gastroparesis, potential thyroid C-cell concerns, and pancreatitis risk in susceptible individuals.
FLGR-242 is aggressively anabolic, and the discipline required to use it properly may be too much for some individuals.
(BY THE WAY: You should ONLY use this peptide subcutaneously, as an intramuscular injection causes agonizing pain that can last two to three days)
It should be started at a low dose, in the range of 1.25 to 2.5 mg weekly, and cycled for 12 to 24 weeks on followed by 6 to 12 weeks off to reset receptor sensitivity.
