Except what’s written on their prescription slip is worlds apart:

  • Man #1 gets permission to use Semaglutide once-weekly, along with an informational pamphlet and a follow-up appointment in 6 months

But before you read another word, please keep in mind this is NOT an anti-GLP-1 article.

What I AM against are quick shortcuts. 

The sick-care system has trained millions of people to believe a weekly injection is the ONLY real answer to insulin resistance, and this belief is worth billions of dollars to the companies selling it.

Insulin resistance is overwhelmingly a lifestyle-driven erosion, and it is completely reversible once you understand what actually drives it.

I have spent more than 25 years running metabolic protocols on my own body and working alongside the most forward-thinking clinicians on the planet to generate what I believe to be the answer.

And I lay out the entire fix in Chapter 7 of my newest book, Living Leaner Longer Stronger.

Here is what actually works (and why).

What Insulin Resistance Actually Is

Insulin resistance is a condition in which the body’s cells (particularly in skeletal muscle, liver, and fat tissue) become progressively less responsive to insulin signaling at the receptor level.

Here is the actual mechanism, step by step:

  • Insulin binds to the insulin receptor on the cell surface
  • This triggers a phosphorylation cascade through insulin receptor substrate-1 (IRS-1) and phosphatidylinositol 3-kinase (PI3K)
  • GLUT4 transporters are moved to the cell surface to pull glucose inside

In Living Leaner Longer Stronger, I describe insulin resistance as a sort of erosion.

When you’re consistently over-eating, your cells turn down the volume on insulin’s signal in the same way your ears adjust themselves to a loud room.

When that pathway is chronically impaired, your pancreas compensates by pumping out even more insulin.

Over time, that induced state of hyperinsulinemia compounds into several other diseases (dyslipidemia, hypertension, fatty liver, PCOS, and eventually Type 2 diabetes) if nothing changes.

But as I said in the book, the over-eating is what makes you fat while the insulin is simply reporting on what you did.

Something the carnivore-keto-no-sugar-ever crowd keeps getting wrong.

We’re dealing with a signaling and metabolic flexibility problem, and making this one distinction changes everything about how you solve the problem.

Why the GLP-1 Narrative Misses the Root Cause

GLP-1 agonists work by mimicking glucagon-like peptide-1 (GLP-1), an incretin hormone that slows gastric emptying and suppresses appetite.

They are extraordinarily effective at lowering blood glucose due to the stimulation of insulin secretion, and I have spent years defending them against lazy fear-mongering.

They are not inherently catabolic, and the muscle loss everybody panics about is almost always self-inflicted by under-eating protein and NOT lifting weights.

But here is what the prescribing doctor almost never tells you…

A GLP-1 peptide won’t undo the missed sleep or lower the cortisol load that eroded your insulin sensitivity in the first place.

Some things in life are still your responsibility, no matter what. 

When people escalate their dose fast, stop using their GLP-1 just as abruptly, and get zero behavioral support afterward, the weight comes roaring back.

Sadly, this is the exact setup used in the major clinical trials used.

As I show in my book Metabolic Awakening With GLP-1 Peptides, lifelong use is the exception rather than the rule for people who actually fix the environment surrounding the drug.

Whether you are a man navigating testosterone optimization or a woman navigating perimenopause, leaning on a GLP-1 as your ONLY strategy fat loss while ignoring the other fundamental factors is how you end up lighter without becoming healthier.

The 4 Pillars of Natural Insulin Sensitization

In Living Leaner Longer Stronger, I lay out the fix as a hierarchy ranked in order of actual impact.

1. Strength Training: The Most Underrated Metabolic Drug

Skeletal muscle is the largest glucose disposal site in your entire body, making up roughly 40% of total body mass in a healthy adult.

The more functional muscle you carry, the more places glucose can go without your pancreas having to scream about it.

And population data show higher relative muscle mass tracks directly with lower insulin resistance and lower prediabetes risk.

Resistance training upregulates GLUT4 expression in muscle tissue and improves insulin signaling, while muscle contraction itself drives glucose uptake through insulin-independent pathways.

This is the foundation.

Lift weights three to four times per week, with four being the sweet spot I have recommended since I published The Testosterone Optimization Therapy Bible several  years ago.

Then make sure you’re also moving outside the gym, because walking and standing throughout the day keep glucose disposal running in the background.

2. Sleep: The Recovery Variable Everyone Ignores

I have watched otherwise disciplined men and women sabotage their entire metabolic stack with chronically poor sleep habits.

A single night of partial sleep deprivation is enough to induce insulin resistance across multiple metabolic pathways in healthy adults.

String a handful of those nights together and you have done more damage to your insulin response than the donut(s) you are so proud of avoiding.

The mechanism at play involves:

  • Elevated cortisol
  • Growth hormone dysregulation
  • Increased inflammatory cytokines
  • Disrupted circadian signaling that directly affects glucose metabolism

Six to eight hours of deep, uninterrupted sleep is a metabolic intervention you should implement and fix before adding any compounds to your stack.

3. Dietary Strategy: Precision Over Restriction

Step #1is simple: Get your total energy intake in line with reality, because no amount of carb timing fixes a caloric surplus whose existence you are seemingly unaware of.

The Diabetes Prevention Program proved how powerful this is, cutting progression to Type 2 diabetes by 58% with roughly 150 minutes of weekly activity and a modest 7% reduction in body weight.

I like to employ the Metabolically Flexible Living framework from 30 Days 2 Shredz, which I consider to be a modern-day upgrade to the insulin-controlled lifestyle I first laid out in The TOT Bible.

Here is what it looks like in practice:

  • Source most of your carbohydrates from lower-glycemic, higher-fiber foods like vegetables, tubers, legumes, and fruit.
  • Concentrate those carbohydrates around your training sessions (when muscle is primed to use them instead of store them).
  • Keep protein high to protect muscle and control appetite.
  • Minimize consumption refined seed oils like soybean, canola, sunflower, and safflower that load you with inflammatory omega-6 fats.
  • Prioritize fats from olive oil, avocado, eggs, grass-fed beef, and wild-caught fish.
  • Compress your eating window, as early time-restricted feeding has been shown to insulin sensitivity in men with prediabetes… even without any weight loss.

If you are heavier and less insulin sensitive, 50 to 100 grams of carbohydrates per day is the range I recommend in 30 Days 2 Shredz (while leaner, more muscular and highly active people can handle considerably more).

Most people who think they are “carb intolerant” are simply stuck in one metabolic gear and haven’t tapped into their innate ability to switch between burning glucose and burning fat (i.e. metabolic flexibility).

The worst thing you can do is graze on six small carbohydrate-dense meals a day because someone told you it would “keep your metabolism going.”

That advice was never grounded in any kind of science to begin with.

4. Managing Chronic Stress and Cortisol Load

Cortisol is a glucocorticoid that directly antagonizes insulin signaling.

Chronically elevated cortisol increases visceral fat and impairs IRS-1 signaling to effectively result in insulin resistance from the top down.

And because your hormones operate as one interconnected network, high cortisol suppresses testosterone at the same time… which only accelerates the metabolic slide.

Stress is a hard biochemical input with measurable downstream consequences.

But don’t freak out, as there are evidence-based practices with documented cortisol-lowering effects that include:

  • Zone 2 aerobic training for 150-180 minutes per week.
  • Breathwork such as cyclic sighing or box breathing, which I treat as the on-ramp to a real meditation practice in Living Leaner Longer Stronger.
  • Ashwagandha (Withania somnifera), which produced a significant cortisol reduction in a randomized, placebo-controlled trial.
  • Deliberate cold exposure to build up autonomic nervous system resilience.

Metformin and Dihydroberberine: Metformin remains one of the most underrated longevity drugs on the planet, and it deserves an honest conversation with a knowledgeable physician if your markers warrant it.

If a prescription is not an option, Dihydroberberine is the closest over-the-counter substitute and I prefer it over standard berberine HCL as it causes far less gastric distress.

Berberine activates AMP-activated protein kinase (AMPK), the same energy-sensing enzyme Metformin targets, and multiple meta-analyses have confirmed its effect on fasting glucose and HbA1c.

Magnesium: Magnesium is a required cofactor for insulin receptor signaling, and an estimated 48% of Americans do not get enough of it from food.

Supplementing with 300-400 mg of magnesium glycinate or threonate daily is a low-risk yet high-return intervention.

Alpha-lipoic acid: Alpha-lipoic acid (ALA) is a mitochondrial antioxidant that aids glucose disposal, which is why I use it around higher-carbohydrate meals and have it built into the BioRecharge formula.

Inositol: Myo-inositol and D-chiro-inositol act as secondary messengers in the insulin signaling cascade, and they are especially relevant for women dealing with PCOS and insulin-driven hormonal disruption.

ATX-304 and BioAmpMAX: ATX-304 is an AMPK activator I use for mitochondrial energy metabolism, glucose control, and insulin sensitivity, and I cover the mechanism in my breakdown of all the known ATX-304 benefits.

My Small Molecule Cheat Sheet runs BioAmp (100 mg ATX-304 per capsule) from one capsule in the morning up to two to three capsules twice daily, for 8 to 12 weeks followed by an equal time off.

The Biomarkers You Need to Track

Vague claims about “feeling better” mean nothing without objective data, so here’s a quick list of what to track and optimize:

Fasting glucose 70 to 85 mg/dL
HbA1c 4.8 to 5.3%
Triglycerides Below 80 mg/dL
Triglyceride to HDL ratio Below 1.0

Fasting insulin is the single most important number on this list, yet it ends up being the one your doctor almost never orders.

A HOMA-IR above 1.5 suggests early insulin resistance, and anything above 2.0 should be addressed immediately.

The triglyceride to HDL ratio is one of the most reliable and underutilized proxies for insulin resistance available on a standard lipid panel, and anything above 2.0 is a strong signal of metabolic dysfunction.

Your conventional doctor will likely tell you your numbers are “fine” when fasting insulin is 12 and HbA1c is 5.6%.

When in reality you’re dealing with early-stage metabolic dysfunction… it just simply hasn’t crossed a pharmaceutical billing threshold yet.

If you can, find an optimization-minded physician who reads these numbers the same way I just described them to you.

Safety and Contraindications

Nothing in this article is medical advice, and none of it replaces a real relationship with a qualified physician who understands health optimization and goes beyond mere disease management. 

That being said, here are four warnings I believe are most relevant in the context of reversing insulin resistance.

  1. If your fasting glucose is already above 126 mg/dL or your HbA1c is above 6.5%, you are in clinical diabetic territory and you require direct medical supervision before making significant dietary or supplementation changes.
  2. DO NOT abruptly eliminate carbohydrate consumption if you are already on insulin or sulfonylurea medications without physician oversight, as rapid drops in blood glucose can be dangerous.
  3. Berberine and Dihydroberberine can interact with certain medications and should not be stacked with Metformin without careful monitoring, due to the fact their glucose-lowering effects are additive.
  4. Cold exposure and high-intensity exercise are powerful tools, but they are stressors that must be earned progressively (especially if you have cardiovascular risk factors).

Own Your Insulin Before It Owns Y ou.

Insulin resistance is not a permanent condition, nor is it the result of a pharmaceutical deficiency.

It is the cumulative result of a metabolic environment quietly eroded by several factors: Overeating (emphasis on hyperpalatable processed foods), poor sleep, chronic stress, sedentary living, , and lost muscle tissue.

Fix the environment and restore your internal signaling by getting the foundational pillars in place.

Only then can a GLP-1 peptide become an accelerant… and even then, plenty of people discover they never required one in the first place!

They also discover they have more power over your metabolic future than the sick-care system will ever tell you.
