It quietly dismantles the cardiovascular system, the nervous system, the hormonal terrain, and overall metabolic health of men who feel completely fine right up until they don’t.

Despite being one of the most reversible drivers of accelerated aging, it nevertheless remains the most underdiagnosed and consequential.

And I AM telling you inflammaging is 100% reversible as someone who has been tracking his own inflammatory biomarkers for over two decades.

I’ve also have the privilege of collaborating with world-class clinicians, while watching thousands of men in my community mismanage this issue.

What Is Inflammaging and Why Should You Care

Inflammaging is a term coined by immunologist Claudio Franceschi in his original evolutionary framework to describe the chronic yet low-grade inflammatory state that develops with advancing age.

It is not the acute inflammation you experience when you cut your finger or fight off a virus, since that kind of inflammation is protective and self-resolving.

Inflammaging is the exact opposite: Systemic in nature while never fully resolving on its own. 

This persistent inflammatory background noise is now linked to virtually every condition we associate with aging: Cardiovascular disease, type 2 diabetes, neurodegeneration, cancer, sarcopenia, and much more.

I break down this exact cascade in my newest book, Living Leaner Longer Stronger, where I walk through how chronic inflammatory inputs from a wide variety of lifestyle habits activate nuclear factor kappa B (NF-kB).

An acronym you should keep in the back of your mind as it is THE master transcription factor governing inflammatory gene expression.

Once NF-kB translocates to the nucleus, it upregulates pro-inflammatory cytokines such as TNF-alpha, IL-6, and interleukin-1 beta (IL-1β).

All three of them suppress growth factor activity and drive mitochondrial dysfunction through increased reactive oxygen species.

As we age, our immune system shifts toward a pro-inflammatory phenotype driven by several converging forces.

Senescent cells accumulate and refuse to die, instead secreting inflammatory compounds through what’s called the senescence-associated secretory phenotype (SASP).

Sex hormones decline, particularly testosterone and estradiol, both of which normally exert direct anti-inflammatory regulatory effects.

Gut microbiome degradation and increased intestinal permeability flood systemic circulation with bacterial endotoxins like lipopolysaccharide (LPS).

Mitochondrial dysfunction generates excess reactive oxygen species that further activate inflammatory pathways, which is exactly why I consider mitochondrial health to be foundational.

But as you’re about to discover, each and every one of these mechanisms is modifiable.

Why Mainstream Medicine Completely Misses This

Your conventional physician is trained to respond to acute clinical events.

They don’t receive a paycheck for detecting and reversing subclinical biological dysfunction that has been silently progressing for a decade.

Furthermore, a standard annual blood panel will tell you almost nothing about your current inflammatory status.

  • A basic CRP (NOT high-sensitivity CRP) will miss low-grade elevations entirely.
  • Inflammatory cytokines like interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) are almost never ordered in routine care.
  • Homocysteine, fibrinogen, ferritin, and oxidized LDL are also absent from a standard panel.

The optimization-minded physician thinks differently, looking for patterns across a broader panel of biomarkers and intervening before pathology is established.

If your doctor is not running the markers I AM about to reveal, find one who will and start with the best lab tests I already outline for exactly this purpose.

How to Actually Measure Inflammaging

Most biohackers and optimization enthusiasts make the critical mistake of assuming one marker is sufficient to tell the whole story about what’s happening on the inside. 

WRONG — you need a comprehensive panel that shows you everything.

The Core Inflammatory Biomarker Panel

hs-CRP (High-Sensitivity C-Reactive Protein), your starting point, is produced by the liver in response to IL-6 signaling.

An optimal reading is anything under 0.5 mg/L, while a reading of >1.0 mg/L warrants investigation.

The AHA/CDC risk framework flags anything above 3.0 mg/L as high cardiovascular risk, which lines up with the same 3.0 mg/L ceiling I have my clients monitor while using therapeutic testosterone.

IL-6 (Interleukin-6) is a key upstream driver of CRP production and a direct mediator of systemic inflammation (often elevated before CRP even moves), with an optimal reading falling under 1.5 pg/mL.

Fibrinogen is a clotting protein that doubles as an acute-phase inflammatory marker, with elevated levels directly leading to an increased risk of cardiovascular disease.

Homocysteine signals oxidative stress and endothelial inflammation when elevated.

An optimal reading falls under 7 µmol/L, ideally within the 4.4 to 10.8 µmol/L range instead of the loose “under 15” most labs report as normal.

Ferritin is often overlooked as an inflammatory marker, with levels above 200 ng/mL in men being a common sign of chronic low-grade inflammation.

Oxidized LDL is the actual atherogenic particle, given that standard LDL panels tell you almost nothing about cardiovascular inflammatory risk.

Fasting insulin and HOMA-IR round out the panel, since insulin resistance is both a cause and a consequence of inflammaging.

Run this full panel at least twice per year if you’re serious about tracking your inflammatory trajectory.

The Most Common Mistakes I See

I’ve watched thousands of men in this community chase anti-inflammatory solutions while completely ignoring the root causes of inflammaging

Here are the most common ways in which men are self-sabotaging their results:

  • They take curcumin and fish oil while eating ultra-processed food every day (supplements CANNOT outrun a pro-inflammatory diet)
  • They ignore sleep quality, with chronic sleep deprivation ranking among the most potent drivers of inflammaging I know of as poor sleep measurably elevates IL-6 and TNF-α.
  • They possess sub-optimal testosterone levels, which is an unforced error as total testosterone is strongly associated with elevated hs-CRP and IL-6 through direct anti-inflammatory effects lost when androgen receptor signaling falls off.
  • They stop measuring their biomarkers altogether (you cannot manage what you do not measure)

How to Actually Reverse Inflammaging

A true reversal of inflammaging requires a multi-vector approach that addresses the root biological drivers instead of being fixated on downstream symptoms.

Foundational Non-Negotiables

  1. Optimize Your Diet at the Mechanistic Level

Eliminate industrial seed oils, as linoleic acid overload directly drives cyclooxygenase (COX) pathway inflammation.

Eliminate ultra-processed foods altoghether.

Build your diet around nutrient-dense animal proteins, low-glycemic vegetables, and quality fats (i.e. minimally processed whole foods)

These three action steps will move your hs-CRP in the right direction.

  1. Fix Your Sleep Architecture

Deep slow-wave sleep is when your body clears inflammatory metabolic waste through the glymphatic system.

Seven to nine hours of high-quality sleep should be treated as a biological requirement for inflammation control.

  1. Optimize Your Hormones

Low testosterone, low estradiol, and low DHEA all correlate with elevated inflammatory markers.

The first one is most important (duh!) as it suppresses inflammatory cytokines like TNF-alpha and IL-6 while raising the production of anti-inflammatory cytokines.

Getting your hormones into optimal ranges is mandatory for anyone serious about reducing inflammaging.

For the women reading this, testosterone optimization plays a documented role in managing inflammatory load in perimenopausal and postmenopausal women.

This is a HUMAN issue and not one exclusive to gender.

  1. Exercise Intelligently

Moderate-intensity resistance training and zone 2 cardio are anti-inflammatory in nature.

But don’t take things too far: Chronic overtraining without adequate recovery is profoundly pro-inflammatory, so train hard and recover harder.

Advanced Interventions Worth Investigating

Omega-3 fatty acids (EPA/DHA) have substantial evidence for reducing IL-6 and CRP at high doses (3 to 4 grams of combined EPA/DHA daily is what I recommend).

Berberine activates AMP-activated protein kinase (AMPK), which directly suppresses NF-kB signaling and reduces inflammatory output (and so does Metformin).

Low-dose naltrexone (LDN) modulates toll-like receptor 4 (TLR4) signaling and has meaningful anti-inflammatory and immune-modulatory effects.

Rapamycin, an mTOR inhibitor used intermittently at low doses, is emerging as one of the most compelling longevity and anti-inflammaging interventions available.

One landmark human trial has already demonstrated that short-course mTOR inhibition measurably improves immune function in older adults.

Peptides such as BPC-157 and Thymosin Beta-4 (TB-500) are among the ones I have personally used with great success for maintaining a low-inflammation environment.

Their mechanisms involving vascular endothelial growth factor (VEGF) upregulation and tissue repair are directly relevant to reducing inflammatory burden at the local and systemic level.

Senolytics, particularly compounds like dasatinib and quercetin that selectively eliminate senescent cells, are one of the most exciting frontiers in directly addressing the cellular root cause of inflammaging.

A first-in-human senolytic trial already confirmed that a single intermittent course of dasatinib and quercetin can measurably reduce senescent cell burden and circulating inflammatory markers in human subjects.

Not everything in the anti-inflammaging toolbox is appropriate for everyone.

Rapamycin at higher doses can suppresses immune function, even though intermittent low-dose protocols appear to be perfectly safe.

Aggressive caloric restriction, while anti-inflammatory, can be destructive to muscle mass and hormonal output if it is not managed carefully.

If your hs-CRP is above 10 mg/L, rule out any active infection or acute injury before attributing the reading to chronic inflammaging.

And finally..

ALWAYS retest biomarkers after implementing any intervention(s), since assuming a protocol worked without verification is guesswork at best.
