In June 2025, a peptide most Americans have never heard of made history.
China’s regulators cleared Mazdutide for chronic weight management, making it the first dual GLP-1/glucagon receptor agonist ever approved for any indication anywhere in the world.
The New England Journal of Medicine had published its Phase 3 data just weeks before.
The Western mainstream barely paid attention because it was still laser-focused on Semaglutide.
I have been tracking this molecule since its earliest trial data under the code name IBI362, and everything I laid out about dual receptor agonism in my book Metabolic Awakening With GLP-1 Peptides is finally playing out in the human results.
What this compound can do for energy expenditure and liver health deserves the full attention of anyone with fat loss front-of-mind.

What Mazdutide Actually Is
Mazdutide, also known as IBI362 or OXM3, is a dual GLP-1/glucagon receptor agonist developed by Innovent Biologics in partnership with Eli Lilly.
It is a once-weekly subcutaneous injection engineered to activate two distinct receptor pathways at the same time.
- GLP-1 receptor activation suppresses appetite, slows gastric emptying, improves insulin sensitivity, and reduces food intake.
- Glucagon receptor activation raises resting energy expenditure and promotes fat oxidation while pushing the liver toward lipolysis instead of fat storage.
Combined, these two pathways attack obesity from opposing sides: You eat less AND you burn more.
In other words…
Activating the glucagon receptor (GCGR) alongside the glucagon-like peptide-1 receptor (GLP-1R) creates a fundamentally different metabolic effect than GLP-1 activation alone.
This dual mechanism is why the weight loss data on Mazdutide is turning heads in the peptides and longevity research communities.
The Mechanism: Why Dual Receptor Activation Matters
GLP-1R activation works primarily through cyclic AMP (cAMP) signaling in the hypothalamus and brainstem to reduce hunger signals, and in the pancreas to stimulate glucose-dependent insulin secretion.
Glucagon receptor activation — through the same cAMP pathway operating in different tissue targets — increases thermogenesis in brown adipose tissue (thereby elevating basal metabolic rate) and accelerates hepatic fat oxidation,
The result is a compound simultaneously reducing caloric intake AND increases caloric expenditure, albeit we don’t have an exact number figure for the latter benefit.
Single-target GLP-1 drugs like Liraglutide and Semaglutide only address the intake side via appetite suppression.
Mazdutide’s design derives from oxyntomodulin, a naturally occurring gut peptide that activates both receptors and has been studied in humans for more than two decades.
And is also known to reliably reduced food intake and body weight.
The task faced by the pharmacology-focused scientists in the pharmaceutical companies was to stabilize that dual activation into a long-acting weekly formulation without losing the therapeutic signal.
If we take the trial data we’re about to see at face value, it’s fair to say this task was successfully completed.
Mazdutide Weight Loss: What the Clinical Data Shows
In the Phase 2 trial conducted in overweight/obese Chinese adults, 24 weeks of Mazdutide produced average body weight reductions of 6.7% at 3mg, 10.4% at 4.5mg, and 11.3% at 6mg (while the placebo group actually gained weight).
Then came the GLORY-1 Phase 3 trial in 610 adults, and the results got even better.
At 48 weeks, the 4mg dose delivered 12.05% average weight loss and the 6mg dose delivered 14.84%, with over half of the 6mg group losing at least 15% of their body weight.
For context: Semaglutide at 2.4mg weekly produced 14.9% weight loss over 68 weeks in the STEP 1 trial.
Mazdutide matched that headline number 20 weeks before Semaglutide did and at a lower molar dose… while weight loss was STILL trending downward when the study ended!
A separate Phase 2 trial in adults with a BMI of 30 or higher has since been published, and the higher dose (9mg) is already moving through China’s review process for moderate to severe obesity.
Additionally, GLORY-1 also showed favorable effects on:
- Fasting blood glucose and HbA1c reduction
- Triglyceride, total cholesterol, and LDL cholesterol improvement
- Blood pressure and serum uric acid reduction
- Waist circumference reduction, which matters enormously if you understand how visceral fat accelerates aging
I left the effects on liver out of the list above because it deserves to be highlighted on its own.
Among GLORY-1 participants who started with liver fat content of 10% or more, the 6mg dose reduced liver fat by a staggering 80.24% at 48 weeks (vs. 5.27% for placebo).
Easily explained by the fact glucagon receptor activation directly drives hepatic fat clearance, something GLP-1 receptor agonism alone cannot accomplish with the same efficiency.
Mazdutide Dosage: What the Research Indicates
The trial data hands us a clean dose-response map.
Meanwhile, two decades of real-world GLP-1 community experience compliments what’s observed in a clinical setting by telling us how to successfully use Mazdutide.
Here’s what we know about the subcutaneously-injected doses studied in clinical trials:
- 3mg weekly – meaningful weight loss with the mildest side effect profile
- 4mg to 4.5mg weekly – substantially increased efficacy with a moderate increase in gastrointestinal (GI) side effects
- 6mg weekly – the strongest published weight loss response, with the highest rate of GI adverse events
- 9mg weekly – under investigation for moderate to severe obesity
The dose-escalation strategy used in the trials mirrors what works across the entire GLP-1 class of medications: Start low, titrate slowly, let the gut adapt before each increase.
Aggressive dose escalation continues to be the #1 mistake I see people make with these compounds.

Your body requires time to adjust receptor sensitivity and gastric motility to the new signaling environment
Speed through that process will guarantee weeks of nausea, vomiting, diarrhea, and the kind of misery infamous for the high rates of discontinuation among GLP-1 peptide users.
My standing philosophy applies here just as it does with every compound in this class:
The lowest effective dose wins, and that usually entails microdosing a GLP-1 over chasing the highest trial dose.
Like I always say: Work with an optimization-minded physician to build a titration schedule that respects your individual biochemistry.
Mazdutide vs Semaglutide: The Real Comparison
This is the question I get asked constantly, so here is the direct answer in table format:

Mazdutide has the mechanistic advantage for pure metabolic optimization and liver health, but the sick-care system’s approval timelines have never dictated what serious biohackers pay attention to.
Neither compound should be used without a structured protocol that incorporates resistance training, adequate protein intake, proper supplementation, and consistent monitoring of YOUR response to the drug.
Lean mass loss is a documented risk across the GLP-1 class, and Mazdutide is not exempt from that concern.
If you are using any compound in this category without lifting heavy and eating 1 gram per pound of lean body weight or more, you are losing muscle tissue alongside the fat.
Bodybuilders figured out these protein targets decades before the clinical literature caught up, and progressive physicians now recommend exactly what that community has practiced all along.

Side Effects and Safety Profile
Mazdutide’s side effect profile looks very similar to every other GLP-1 based compound.
The most common adverse events reported in trials include some of the following:
- Nausea (most frequent, especially during titration)
- Diarrhea
- Vomiting
- Constipation
- Decreased appetite (expected and therapeutic)
- Injection site reactions (mild, transient)
- A small average heart rate increase of roughly 2.6 beats per minute at 48 weeks
These events were mostly mild or moderate in the trials… and dose-related too, which is exactly why titration speed matters so much.
The glucagon receptor component raises a theoretical concern worthy of your awareness:
Glucagon historically raises blood glucose, but in the context of GLP-1 co-activation, the insulin-stimulating effects of GLP-1R activation counterbalance this pathway.
Hypoglycemia risk appears low in non-diabetic users, also consistent with the glucose-dependent nature of GLP-1 mediated insulin release.
WARNING: DO NOT use Mazdutide, or any compound in this class, if you have a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia type 2 (MEN2).
This class-wide caution comes from rodent data showing C-cell hyperplasia with GLP-1 receptor stimulation, and this is a scenario where it’s better to be safer than sorry
Here are some additional contraindications and cautions that may impact your decision to use or avoid Mazdutide:
- Active pancreatitis or history of pancreatitis
- Severe gastrointestinal disease
- Pregnancy or breastfeeding
- Concurrent use of other insulin secretagogues without monitoring

- Structured progressive resistance training, minimum 3x per week
- Zone 2 cardio as a metabolic multiplier, not a crutch
- High protein intake to defend lean mass during the caloric deficit
