If you’ve spent enough time in the NAD+ rabbit hole, you may have found yourself taking three separate compounds in 3 separate ways.

$90 on a jar of NMN powder.

$40 for a bottle of NR capsules.

Hundreds of dollars for a single afternoon glued to a chair so you can receive NAD+ injections through an IV drip.

Aside from the high price tag, all three products sell you on the same promise.

But what REALLY matters here is what happens to the molecule after it enters your body… the one thing that determines whether what you’re doing will work.

The NAD+ space is one of the most overhyped and under-clarified corners of the entire longevity market.

Brands are making a fortune while most buyers have no idea which form actually raises NAD+ inside the tissues in a meaningful way.

Nor do they know what delivery route they should be using.

I have been running NAD+ optimization protocols for years alongside some of the top longevity clinicians and researchers in the world.

And today, you’re going to learn everything I know to be true about this molecule.

Why NAD+ Matters in the First Place

Nicotinamide adenine dinucleotide, or NAD+, is a coenzyme found in every single cell in your body.

It is the master regulator of cellular energy production, but also plays pivotal roles in DNA repair and longevity signaling.

Without adequate NAD+, your mitochondria cannot efficiently run the electron transport chain.

Consequently, your sirtuins cannot perform their epigenetic repair functions and your PARP enzymes cannot properly respond to DNA damage.

NAD+ production declines with age in human tissue, and by your mid-40s you have typically lost +50% of what you carried in your 20s.

This has been shown by extensive research to be one of the central mechanisms driving accelerated aging, metabolic dysfunction, neurodegeneration, and collapsing resilience.

The review that put NAD-boosting molecules on the map for serious researchers came out of David Sinclair’s group in 2018, and the in vivo evidence has only gotten stronger since then.

Restoring NAD+ can be a legitimate longevity strategy when it is employed correctly.

And it sits in the foundational tier of the hierarchy I laid out when I compared it against Klotho and Epitalon, right after hormones but before anything more exploratory in nature.

The Three Primary Delivery Methods Explained

Let’s break down what you are actually getting with each of the 3 options.

NAD+ Injections (IV and Subcutaneous)

Direct NAD+ administration, intravenously or subcutaneously, skips the issue of precursor conversion question.

In other words, you are delivering the finished molecule into systemic circulation.

IV protocols typically run from 250 mg to 1,000 mg over several hours and have been used by anti-aging clinics for the purposes of addiction recovery and neurological repair.

Subcutaneous injections are the more practical version for obvious reasons and they are the ones I actually use on myself. 

My Peptide Cheat Sheet protocol calls for 100 mg subcutaneously in the morning (from a 500 mg vial reconstituted with 2 ml of bacteriostatic water), two to three days per week, 

My wife Monica runs women at 50 to 100 mg, two to three times per week, on a “three months on and one month off” basis.

The biggest advantages of injecting are going to be speed and systemic availability.

The biggest limitation is this: NAD+ is a large charged molecule that does not freely cross cell membranes.

Previous work on precursor pharmacology shows extracellular NAD+ is largely broken down by surface enzymes into smaller intermediates like NMN and NR, which are what actually get taken up and rebuilt into NAD+ inside the cell.

What this means is the expensive IV bag you’re putting into your veins is partially functioning as a very fast yet very costly precursor delivery system.

That’s not to say it is a worthless delivery method, but just understand you’re not “injecting pure NAD+ straight into your cells” as some people would have you believe.

NMN (Nicotinamide Mononucleotide)

NMN sits one step away from NAD+ on the salvage pathway, and gets converted directly into NAD+ by the enzyme NMNAT.

It’s  the shortest route of any oral precursor to NAD+, hence why NMN gets the attention it does.

The landmark human trial from Yoshino and colleagues in Science found oral NMN improved muscle insulin sensitivity in postmenopausal women with prediabetes, which in and of itself represents a real tissue-level functional outcome.

Chronic supplementation in healthy older men has also been shown to elevate blood NAD+ levels and alter muscle function.

And earlier Japanese work established single oral doses were safe and well tolerated in healthy men

(NOTE: This is the same study most blogs cite incorrectly as a NAD+ elevation trial, so be careful with what you read and absorb from the Internet). 

Sublingual and liposomal formats have gained considerable traction over the years as bypassing first-pass liver metabolism improves systemic availability compared to what you’re getting with a standard capsule.

Having used NMN extensively on myself, I can confidently say it is one of my core precursor strategies.

NR (Nicotinamide Riboside)

NR is a form of vitamin B3 that also feeds the salvage pathway, but it has to be phosphorylated into NMN first before it can be converted into NAD+.

That extra enzymatic step matters far more in older people whose enzyme activity is already declining.

Trammell and colleagues established in their published data in Nature Communications that NR is uniquely and orally bioavailable in both mice and humans, raising whole blood NAD+ in a dose-dependent fashion.

A randomized trial then demonstrated chronic NR supplementation is well tolerated and elevates NAD+ in healthy middle-aged and older adults.

NR has the longest and deepest human trial record of any oral precursor, which is why I recommended 250 to 300 mg daily in the morning in my newest book Living A Fully Optimized Life.

However, one thing I would like to direct your attention toward is a 2023 review asking what is really known about NR’s utility in humans.

As of this writing, there is no well-powered human head-to-head trial of NMN against NR.

Which means anyone telling you one definitively beats the other per milligram is extrapolating from what’s available in the existing data and making an educated guess. 

My preference for daily-use NMN comes from the shorter conversion path and from my own tracked results.

Again, this is a personal preference rather than a proven verdict.

What Most People Get Wrong About NAD+ Supplementation

People love to argue about which of the 3 molecules is best without understanding that intracellular NAD+ is the only metric that matters.

Blood NAD+ is not a reliable proxy for what is happening inside your brain, your heart, your liver, or your skeletal muscle for the reason that raising serum NAD+ is relatively easy.

Raising NAD+ in the target tissue itself is the actual challenge.

That’s why delivery route, formulation quality, the dose used, and the overall dosing strategy (timing, stacking, etc.) are critical to take into account.

What also matters protecting the NAD+ you already have, because apigenin is an inhibitor of CD38, the enzyme that consumes NAD+ at an accelerating rate as you age.

Boosting production while ignoring consumption is like filling a bucket without checking for holes first.

The problem primarily lies with the fact most brands sell you a single capsule and refer to it as a comprehensive NAD+ protocol.

(NOTE: There is one exception to this generalization, which I AM going to introduce you to in the next section)

What I Actually Use and Recommend

The right compound to take, and how to take it, will ultimately be a conversation to be had between you and an optimization-minded physician who understands this science.

What I can share with you is what my own protocol looks like and what I have watched work across thousands of people in our community.

Subcutaneous NAD+ at 100 mg, two to three mornings per week, is one of the highest-leverage tools I have used for cognitive performance and overall recovery.

NMN at 500 mg to 1,000 mg daily taken sublingually (or liposomal when I can get it), is my foundational oral precursor.

I have cycled between NMN and NR depending on the season for many years, and both have earned their place in my toolbox.

But nothing listed above replaces the basics… so get your mitochondrial health fundamentals handled and make sure the rest of your mitochondrial support supplements are actually in place before you spend serious money on precursors.

Female-specific NAD+ research, particularly around the metabolic shifts of menopause, is an area where I expect meaningful development over the next five years.

Safety, Risks, and Contraindications

NAD+ precursor supplementation has a strong general safety profile across the existing literature, but there some things worth knowing about…

  • Some people get nausea, flushing, or GI discomfort at higher NMN doses (start low and titrate up the dose slowly)
  • IV NAD+ can produce real discomfort during infusion (including chest tightness and nausea), which is expected at higher infusion rates and is managed by slowing the drip.
  • Anyone with an active cancer or a pre-cancerous state should discuss NAD+ protocols carefully with their oncologist, given that cancer cells depend on NAD+ for energy metabolism in the same way that healthy cells do.
  • If you are on medications affecting PARP activity and/or sirtuin signaling, work with a physician before starting a high-dose NAD+ protocol.

Lastly, learn how to evaluate peptide quality before you put anything from anyone into your body.

Sterility and purity requirements for any injectable are NON-NEGOTIABLE, so make sure you ONLY procure NAD+ from suppliers providing batch numbers and third-party certificates of analysis.

The Bottom Line on NAD+ Optimization

The real question worth answering is what your body requires.

Along with the right dose and delivery method you use (plus anything you’re stacking) to actually increase intracellular NAD+ levels.

Injections give you the fastest systemic rise and the highest acute impact, which is why they are often a part of aggressive anti-aging protocols.

NMN gives you the shortest oral route, with growing human evidence and a strong mechanistic case continuing to support its use.

NR gives you the most extensively studied oral option with the longest safety record, but at the cost of one extra conversion step.

A truly optimized NAD+ protocol, therefore, revolves more around an overall strategy than a single product.  

And this strategy sits downstream of the foundational pillars: Hormones, training, sleep, metabolic health, and more.

So make sure you fix those first so you don’t make the common mistake of pouring expensive precursors into a broken system.
